JOB SUMMARY:

A Behavioral Health Clinician serves as a member of the Behavioral Health Team in the outpatient clinic at the Wind River Family & Community Health Care Center.  Providing behavioral health treatment to individuals, families, and groups.

DUTIES AND RESPONSIBILITIES:

Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations

Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff

Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care as required

Participates with other medical professionals in educating the community and medical staff on mental health issues

Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice

Collaborates with WHRFCHC and other Tribal departments for continuity of care

Other duties as assigned to maintain/enhance the operation of the Behavioral Health Department

QUALIFICATIONS:

Masters’ level education or higher in Social Work, Professional Counseling, Marriage and Family Therapy

Current licensure, or the ability to obtain the appropriate licensure within a reasonable amount of time (under the supervision of personnel currently in place) and has already passed the test required for licensure

Must possess a valid driver license and be eligible for coverage under WRFCHC auto insurance policy. 

Must be able to pass a criminal background check under the Indian Child Protection and Family Violence Prevention Act (P.L.101-630).

Current BLS Certification or be able to obtain within 60 days of hire.

JOB OVERVIEW:

Psychologists will assist the Behavioral Health Department by providing both psychological evaluations and consulting to behavioral health clinicians as well as provide individual and group mental health therapy.

DUTIES AND RESPONSIBILITIES:

Responds to referrals from the Behavioral Health and Medical Providers
Provide competent and prompt psychological evaluations to children, adolescent, and adult patients to facilitate the identifying of needed resources and determining treatment recommendations.
Provide psychological testing within their scope of practice and completes reports in timely manner.
Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations.
Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff.
Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care.
Participates with other medical professionals in educating the community and medical staff on mental health issues.
Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, update clinical record, including progress notes and other relevant information, and enters the necessary information into the electronic health record system.
Collaborates with WHRFCHC and other Tribal departments for continuity of care.
As a team member develop productive and cooperative working relationships with health care providers within the facility as well as allied healthcare and social service providers.
Performs other duties as assigned to maintain and enhance the operation of the program.

QUALIFICATIONS:

PhD in psychology or PsyD with current independent licensure.
Must be committed to providing comprehensive health care to the individual, family, and community.
Must maintain strict confidentiality and high ethical standards in performing the position.
Must be able to pass a criminal background check under the Public Law 101.630-Indian Child Protection Act and Family Act.
Must have the ability to work in a variety of settings with culturally diverse families and communities, with the ability to be culturally sensitive and culturally appropriate.

JOB SUMMARY:

The Custodian will be responsible for maintaining cleanliness and orderliness within Wind River Family and Community Health Care’s (WRFCHC) facilities, ensuring a safe and healthy environment for all occupants. This position involves rotating at different facilities as needed to ensure infection prevention across all WRFCHC facilities. 

DUTIES AND RESPONSIBILITIES:

Perform all assigned tasks in a professional manner to reflect the highest integrity of the department to ensure the continuity and efficiency of patient care and daily operations.

Perform routine cleaning of exam rooms, nursing stations, offices, clinical areas, waiting areas, lobbies, lounges, rest rooms, corridors, and stairways in a clean, neat, and sanitary manner.

Perform routine cleaning of high traffic areas of walls, cabinet surfaces, tables, stands and any other furniture and areas as directed.

Perform cleaning and maintenance of all floors: carpet, tile, brick, and concrete flooring.

Completes trash, biohazardous linens, and sharps container removal to appropriate disposal area.

Maintain inventory of assigned areas cleaning supplies, paper products, and other essential items required to keep all areas functioning.

Utilizes the appropriate chemicals, supplies, and cleaning techniques according to procedure and manufacturers’ recommendation.

Assists visitors with directions whenever requested.

Is trained/cross trained in all facilities to ensure ability to meet workload requirements when staffing shortages and/or vacations reduce staff levels.

Report safety hazards as appropriate.

Replenishes supplies; notifies supervisor or designee when supplies need to be replenished.

Follows all custodial services practices including hand washing techniques, standard precautions, and isolation precautions.

Observe and follow all health and safety regulations at all times.

Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

High School Diploma or equivalent required.

Proven experience in custodial or janitorial work 

Knowledge of chemicals, cleaning techniques, equipment operation, and safety protocols

JOB SUMMARY:

The Dental Director position oversees and manages the operations of Wind River Family and Community Health Care’s dental practice focusing on patient care and staff management ensuring compliance and quality standards are met. This position requires leadership, coaching, organizational, communication, analytical, and problem solving skills.

DUTIES AND RESPONSIBILITIES:

Supervisory:

Develops and writes the policy and procedures and standing operating basis for the Dental  Department in alignment with WRFCHC guidelines.

Participates in organizational meetings to develop new, modify existing and evaluate medical procedures relative to patient care with the objective of constantly improving and increasing the degree of patient care given to clients.

Maintain scheduling and timesheets of Dental staff  and set policy and standards for the department.

Oversees the daily workflow of the Dental department.

Complete Performance Evaluations and corrective action plans (when necessary) for those supervised.

Handles disciplinary action for the Dental staff in accordance with WRFCHC policy.

Make appropriate referrals to other specialists or services as needed.

Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice.

Review monthly billing totals and find opportunities to help increase collections.

Maintains a staff of sufficient numbers to effectively provide quality care for patients of all ages.

 Trains new personnel.

Provides opportunity for development and professional growth of staff.

Other duties as assigned, to maintain and enhance the operation of the department. 

Clinical:

Makes clinical and radiographic examinations of the oral hard and soft tissues and a diagnosis of pathological or irregular conditions of the oral cavity and maxillofacial area.

Performs endodontic, prosthodontic, periodontic, operative, preventative, and restorative dentistry within the scope of practice and ability.

Performs tooth extractions and refers as needed to the oral surgeon.

Performs root canal therapy within scope of practice.

Treats scheduled and emergent patients as needed.

Ensures accurate input of patient demographics into Dentrix Database.

Informs patients about good dental health practices.

To maintain a sterile/disinfected environment in the delivery of oral care. 

QUALIFICATIONS:

Applicants must have a Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) degree from a school approved by the American Dental Association (ADA) or the Commission on Dental Accreditation (CODA) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Applicants must have passed the National Board Dental Examinations (NBDE) which are developed and administered by the Joint Commission on National Dental Examinations (JCNDE) and possess a current, full, and unrestricted license to practice dentistry in the State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Two years of supervisory experience preferred.

Busy, fast-paced outpatient clinic setting that involves a full range of motion, manual, and finger dexterity, and eye-hand coordination.

The dialysis charge nurse is responsible for overseeing the daily operations of the dialysis unit, ensuring high-quality patient care, and compliance with CMS guidelines 

Key Responsibilities:

Supervise nursing staff and dialysis technicians during treatment sessions 

Coordinate patient care plans with nephrologists and interdisciplinary teams

Monitor patient vitals and responses during dialysis procedures

Ensure proper setup, operation, and maintenance of dialysis equipment

Administer medications and assess vascular access sites

Educate patients and families on dialysis procedures and kidney health

Maintain accurate and timely documentation in electronic health records

Respond to emergencies and troubleshoot complications during treatment

Must have Basic Life Support Certification.

JOB SUMMARY:

On-the-job training provided. The patient care technician provides safe and acceptable dialysis under the supervision of a registered nurse. Communicates with patients, families, and the health care team to provide services according to the patient care plan.

DUTIES AND RESPONSIBILITIES:

Performs basic patient care activities.

Observes patients and reports unusual conditions and/or reactions to RN.

Obtain blood samples for laboratory analysis as required to complete physician orders.

Perform duties at all times under the supervision of a Registered Nurse and within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations. 

Use appropriate safety measures, including personal protective equipment as necessary.

Clean, disinfect, and rinse dialysis machines in accordance with policy and procedures.

Prepare machines for hemodialysis treatments. 

Obtain pre and post treatment weights of patients per protocol. 

Safely initiate and terminate dialysis treatments per center protocol, and within scope of practice and as allowed by state regulations. 

Monitor patients during dialysis treatments. 

Mix acids and bicarbonate according to protocol. 

Complete water testing on a dialysis machines at the beginning of each treatment. Complete and document quality control logs throughout the day and after all treatments, as required.

Responds to the patient’s needs with compassion and concern.  

Participate in staff meetings as required, including in-service and continuing education that are offered for certifications. 

Other duties as assigned to maintain and enhance the department. 

QUALIFICATIONS:

High school diploma or GED.

Basic computer knowledge

BLS certification or obtained within  60 days of hire.

Confirmation of ability to distinguish all primary colors. 

A minimum of 3 months of experience in dialysis healthcare, preferred but not required. 

Must be certified under a national certification program. Certification must be obtained within 18 months of hire date as a Patient Care Technician.

Must be able to prioritize multiple details simultaneously. 

Must have analytical and interpretive skills. 

Requires level of short and long-term mental faculties.

DUTIES AND RESPONSIBILITIES:

Provide dialysis treatments as prescribed by physicians, ensuring patient safety and comfort.

Administers prescribed medications while patients are on dialysis in a safe manner and documents in EHR according to state and CDC guidelines.

Regularly check and record patients’ vital signs before, during, and after dialysis sessions. Monitor for any adverse reactions and take appropriate action.

Reviews and analyzes a variety of accounting transactions involving State, Federal, Community funded programs and other funding sources.

Aid in analyzing complex financial data and preparing accurate records and reports.

Manage purchasing card transactions through an external credit card management system.

Proofread and reconcile all accounting records to check for arithmetical accuracy, completeness and compliance of regulations.

Establishes and maintains accurate confidential records and files and communicates effectively both verbally and in written form under the stress of strict deadlines.

Gathers and prepares data and documents for a variety of financial reports.

WORKING ENVIRONMENT:

The provider will see, on average, 14 patients per clinic workday. 

The provider schedule is four 10-hour shifts or five 8-hour shifts per week, Monday through Friday.

The provider will be based in our outpatient clinics. The provider will typically have one CNA and one MA on their team, as well as a shared RN and 4 or 5 provider colleagues working at the same location. Providers may also be expected  to provide care at local nursing homes, schools, children’s homes, homeless shelters or other community based facilities. We try to align providers’ interests and areas of expertise when matching providers to these community needs. 

If the provider is interested they may be able to join our in patient care group which rotates call at our local hospital. Average 3-5 24hr shifts a month. Must be a physician, the local hospital does not credential NP/PA providers. 

The provider will be a part of a rotation pool to staff weekend and evening shifts at our clinic in Riverton. The provider will not be scheduled for more than 6 of these shifts a year. They will be expected to serve as ‘back up call’ for this coverage no more than 4 weekends a year. These shifts are from 12:30-10:30pm Friday through Monday. Details about inpatient hospital duties can be discussed in detail if indicated. If a provider takes on in patient care they are exempt from these expectations.  

Primarily works in a clinical environment, including outpatient exam rooms, treatment areas, and administrative office settings with potential exposure to infectious diseases, bodily fluids, hazardous materials, and sharp instruments. 

The environment can be fast-paced with frequent interruptions, requiring the ability to prioritize tasks, make decisions quickly, and respond effectively to urgent medical needs.

JOB SUMMARY:

This position supports the medical providers and nursing staff to ensure all patients receive quality care.

DUTIES AND RESPONSIBILITIES:

Performs Nebulizer treatments.

Performs oral, topical, intramuscular, subcutaneous, or intradermal administration of non-controlled medications.

Performs immunizations (subcutaneous or intramuscular)

Performs 12 lead electrocardiogram (EKG) and gives report to RN and/or medical provider.

Performs throat cultures and laboratory sample collection, within the CMA’s scope of practice.

Performs Ear irrigation.

Keep exam rooms stocked with adequate medical supplies, maintain instruments, prepare sterilization as required.

Assists with administrative tasks as necessary, such as answering patient telephone calls and scheduling appointments.

Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

High school diploma or GED equivalent

Current certification for a Medical Assistant from accredited program.

Supporting certification such as BLS and other necessary competencies (e.g., ACLS, TNCC, PALS etc.) or obtain within 60 days of hire.

Must be able to demonstrate skills in the practical application of electronic systems and database management software and satisfactory sterile techniques.

JOB SUMMARY:

The MCH Nurse will promote maternal self-care and infant care, assess and monitor maternal physical and emotional changes, foster parent infant relationship, assess and advise on safety of infant environment.  Provide assessment and teaching and foster independence with breastfeeding and monitor infant growth and development.  MCH Nurse develops, evaluates and revises patient care goals in conjunction with patient and family. 

Communicates with MD, family and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.  Maintain effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives. 

DUTIES AND RESPONSIBILITIES:

Carries out the Agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function. 

Accesses and evaluates the health care needs of patients and families with consideration regarding physiological, psychological, social and environmental factors. 

Develops a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community and agency resources. 

Implements the care plan and revises it whenever necessary by regularly assessing, observing and evaluating the patient’s condition, needs and response to care, and makes appropriate nursing judgments and decisions for care plan revision.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:

The work environment includes working in an ambulatory clinic, inpatient-hospital visits, home visits, community events (which may require working outdoors) and office.  The incumbent may be required to work with challenging patients.

The position requires standing and sitting for extended periods of time. 

QUALIFICATIONS:

A valid, current, full and unrestricted Professional Nursing License is required. A Bachelors of Science degree from an accredited School of Nursing preferred, and  minimum of 2-4 years of experience working a progressively more responsible nursing position. 

Must demonstrate skills and proficiencies in Maternal Child Health, medical terminology and practices. 

Must complete and maintain certification for lactation consultant and national passenger safety technician within 60 days of employment.

Telemedicine uses videoconferencing equipment to send and receive medical information and pictures. The Telemedicine Nurse facilitates this technology to serve patients who require specialized care in a distant community.

The Telemedicine Nurse will serve as a clinical resource and help to develop, implement, evaluate and design Telemedicine workflows and measures to ensure optimal integration into delivery of care and service. 

DUTIES AND RESPONSIBILITIES:

Provides specialist site with any necessary information.

Sees that requested lab/tests are ordered and results are sent to specialist.

JOB SUMMARY:

Coordinates and provides comprehensive medication management to optimize medication use. 

DUTIES AND RESPONSIBILITIES:

 Manage medication therapy through direct patient assessment to evaluate patient responses to medication therapy, communicate and document patient findings, to make recommendations to care team providers, and to implement and monitor pharmacotherapeutic care plans.

 Interview and counsel patients on the use, storage, cautions and side effects of all medications prescribed. Provides and documents patient education.

Evaluates medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance, adverse drug reaction and cost effectiveness. Analyzes medication regimens to ensure the most effective, least toxic, and most economical treatment plan.

Documents all clinical activities, updates problem lists and records information in the electronic health record when required and maintains pharmacy records as required by law.

 Provides ongoing, comprehensive chronic disease management.

Perform departmental administrative functions, committee involvement, quality assurance activities, statistical analysis, CME presentations, in-services, competency and proficiency reviews and involvement in performance improvement groups. Operate under collaborative practice agreements as approved by the Wyoming Board of Pharmacy, which may include limited prescribing in defined situations. Ability to refer and consult appropriately.

The work requires independent judgement wherein error could result in serious injury or death to a patient.

The work will cover assessment, medical history, development and implementation of care plans, evaluation of the effectiveness of treatment and modification of treatment.

The medical record is carefully reviewed for therapeutic effectiveness and appropriateness including diagnosis, treatment, patient allergy, and drug interaction screening.

 Pharmaceutical care services, drug information and direct patient care, and to manage interventions and therapy between primary physician visits.

 Clinical functions include the selection of appropriate medication for disease state management, monitoring patient outcomes, analysis of adverse drug events, and medication reconciliation.

 Indirect patient care activities include formulary management, quality assurance, medication utilization review, and staff development

QUALIFICATIONS:

 Must have a BS degree in Pharmacy or Pharm.D. and possess a current WY pharmacy license in good standing.

JOB SUMMARY:

Transports patients safely and efficiently for healthcare appointments.

Validates patients’ identity based on established department protocol.

Will be required to assist with patient handling and lifting using appropriate patient safety equipment and appropriate safety procedures.

Remains alert to patient condition and promptly communicates changes to clinical staff.

Responds to all job calls in a timely manner. Completes daily logs, fill out patient transportation referral sheets and turn the documents in by the end of the business day.

Inspects vehicle for supplies, fuel level, oil level, and maintenance defects before pre-trip and post-trips. Reports any issues to the Transportation Supervisor.

Assists with servicing vehicle maintenance and repairs.

Ensures vehicle and equipment are clean and sanitized.

Communicates status updates and location information as required in accordance with department protocols to help facilitate workflow.

JOB OVERVIEW:

Certified Addiction Practitioner (CAP) is a person certified to practice addictions treatment, prevention, intervention, diagnosis, referral and follow up under the supervision of a qualified clinical supervisor.

Completes Addiction Severity Index (ASI) assessment to determine appropriate ASAM Level of Care.

Develops individualized treatment plans with patients based on ASI assessment and ASAM Level of Care Recommendations.

Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.

Completes case management tasks as necessary.

Completes necessary documentation according to CARF accreditation and 3rd party payer standards.

Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.

Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures

Collaborates with WHRFCHC and other Tribal departments for continuity of care.

JOB SUMMARY:

The Licensed Substance Abuse Provider provides professional services to individuals and groups with substance use disorders; these persons may practice independent of supervision, unless provisionally licensed, in which case they will be assigned a clinical supervisor.  This position may be located at either Riverton, Arapahoe or Ethete offices.

DUTIES AND RESPONSIBILITIES:

Provides services based on scope of practice of Licensure including, but not limited to, addition     prevention, crisis intervention, diagnosis, referral, direct treatment, and follow-up treatment which is rendered to individuals, families, groups and/or organizations who are adversely affected by addictions or related disorders. 

Interviews clients and families to complete an assessment tool such as Addiction Severity Index (ASI) and will develop an individualized treatment plan

Applies therapies and interventions to address patients’ cognitive and behavioral impairment by counseling clients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life and making changes. . 

Clinical supervision of Certified Addiction Practitioner (CAP) and Certified Addiction Practitioner Assistants (CAPA), acting as a mentor, if Licensed;

 Provide coverage for Clinical Coordinator when needed. 

Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures.

Stive to maintain client centered practices by integrating family and positive support systems by maintaining professional standards of conduct, and model recovery type behavior.

Other duties as assigned to maintain and enhance the operation of the Department.

QUALIFICATIONS:

Completed a graduate degree program with concentration on addictionology, chemical dependency, or substance abuse from an educational institution accredited by one of the regional or national institutional accrediting bodies recognized by the Council for Higher Education Accreditation (CHEA)

Knowledge of the field of addictions treatment in general by passing a standard examination which may be written or situational, as the board prescribes. 

Knowledge of confidentiality rules and regulations including HIPAA and 42 CFR Part 2

Must have 2 years of continuous sobriety from drugs and/or alcohol.

Must possess a valid driver license and meet insurance standards and maintain insurability under WRFCHCs insurance program.

Must be able to pass a criminal background check under the Indian Child Protection Act and Family Act (P.L.101-630).

JOB SUMMARY:

The Substance Abuse Technician is responsible for the direct supervision of the residents in the Transitional Home.  Substance Abuse Technicians will provide a safe, trauma informed, supportive environment for all residents. 

DUTIES AND RESPONSIBILITIES:

 Accurately report incidents or any behavior that includes significance on provided forms.

Assists and supervises clients with preparation of meals, scheduling appointments, chores, community service and other activities such as providing transportation as needed. 

Communicate difficulties and/or concerns regarding client’s behaviors to the supervisor as appropriate.

Conduct alcohol and drug screening in conjunction with visits and employment as well as compliance room checks and inspection of all resident’s property. 

Ensuring compliance with relevant regulations including certifying agency standards. 

Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health care record (EHR) system. ACCUCARE

Maintain professional standards of conduct, and model positive recovery type behavior.

Adhere to NAADAC Code of Ethics, as well as policies and procedures.

Attend all mandatory training and staff meetings.

Provide coverage assistance to co-workers to ensure supervision of residents at all times. 

Other duties as assigned to maintain and enhance the operation of the Department.

QUALIFICATIONS:

Minimum of High School Diploma or equivalency, prefer 1 year of experience working with substance abuse clients.

 Basic knowledge of substance abuse and recovery.
