JOB SUMMARY:

Serves as a member of the Behavioral Health Team in the outpatient clinic at the Wind River Family & Community Health Care Center.  Providing behavioral health treatment to individuals, families, and groups.

DUTIES AND RESPONSIBILITIES:

Provides intakes, assessments, and treatment planning for patient care. 

Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations.

Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff. 

Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care as required.

Participates with other medical professionals in educating the community and medical staff on mental health issues, including crisis intervention.

Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice. 

Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, update clinical record, including progress notes and other relevant information, and enters the necessary information into the electronic health record system. 

Collaborates with WHRFCHC and other Tribal departments for continuity of care.

Performs other duties as assigned to maintain and enhance the operation of the program.

QUALIFICATIONS:

Masters’ level education or higher in Social Work, Professional Counseling, Marriage, and Family Therapy

Licensed or Provisionally Licensed Behavioral Health Clinician (LPC, LMFT, LCSW, Licensed Psychologist, PPC, PMFT, PCSW)

Current licensure, or the ability to obtain the appropriate licensure within a reasonable amount of time (under the supervision of personnel currently in place) and has already passed the test required for licensure.

Must maintain strict confidentiality and high ethical standards in performing the position.

Must have the ability to work in a variety of settings with culturally diverse families and communities, with the ability to be culturally sensitive and culturally appropriate.

JOB OVERVIEW:

Psychologists will assist the Behavioral Health Department by providing both psychological evaluations and consulting to behavioral health clinicians as well as provide individual and group mental health therapy.

DUTIES AND RESPONSIBILITIES:

Responds to referrals from the Behavioral Health and Medical Providers
Provide competent and prompt psychological evaluations to children, adolescent, and adult patients to facilitate the identifying of needed resources and determining treatment recommendations.
Provide psychological testing within their scope of practice and completes reports in timely manner.
Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations.

JOB SUMMARY:

The Automotive Shop Assistant is responsible for providing automotive interior and exterior cleaning along with disinfection processes.  This position is also responsible for shop cleanliness and monitoring for safety hazards.

DUTIES AND RESPONSIBILITIES:

Clean and disinfect the Mobile Medical Units.

Clean the interior and exterior of Fleet, GSA and customer vehicles.

Use specialized cleaners, waxes, or polish for the specific car.

Use specialized cleaning tools or materials to get the best product.

Maintain the cleanliness of the automotive shop and office and outside perimeters of the shop.

Assist the Mobile Automotive Technicians and Automotive Shop receptionist as needed.

Prove excellent customer service to all Wind River Community Healthcare employees and patients, keeping in mind that their satisfaction comes first.

JOB SUMMARY:

The Lead Youth Prevention Specialist, under the supervision of the Prevention Director, will be responsible for creating a safe and nurturing environment for young individuals. The Lead Youth Prevention Specialist will provide guidance and mentorship to the Activities Coordinators, Youth Prevention Tutors and Cultural Coordinator as well as helping to organize educational, cultural and recreational activities. This position requires leadership skills, attention to detail, and the ability to manage a team.

DUTIES AND RESPONSIBILITIES:

Interact with youth through supervising activities, helping with homework, and programming.

Help coordinate and facilitate structured activities appropriate for different age groups.

Work as a team to help establish a trusting and dependable relationship that can place the youth on a positive path to success.

Help youth thrive through role modeling, encouragement, advocacy, and problem solving.

Support other staff in the care of the youth.

Ensure the safety and whereabouts of youth throughout provided activities.

Assist with routine cleanup.

Maintain and complete necessary documentation.

Supervise and delegate tasks to wellness staff to ensure the wellness center is running smoothly and the youth are being cared for.

Ensures wellness staff are following Policy and Procedures and Standard Operating Procedures. 

Brings concerns raised by youth, families and staff to the Prevention Director and collaboratively work to solve any issues.

Maintains scheduling of wellness staff to ensure there is always enough staff to safely supervise and work with the youth.

Maintain a positive working relationship with the wellness staff to help ensure the team is effective in working with youth.

Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

Minimum of two years’ experience working directly with youth.

Associates degree in youth related field or Human Services.

Clinical

DEPARTMENT: Clinical

JOB SUMMARY:

Functions as a member of the clinical team, assessing nutritional needs of WRFCHC patients. This includes, but is not limited to planning, coordinating, and evaluating the nutritional component of health care services for specific comorbidities and throughout the life span.

DUTIES AND RESPONSIBILITIES:

Works independently and in collaboration with WRFCHC medical team(s) and community outreach programs.
Conducts nutritional assessment and provides Medical Nutrition Therapy (MNT) based on disease process, using the Nutrition Care Process (NCP). Disease processes may include but are not limited to prediabetes, diabetes, gestational diabetes, diabetes during pregnancy, adult obesity, childhood obesity, malnutrition, chronic kidney disease, hypertension, and hyperlipidemia.
Provides diabetes nutrition counseling/education to the patients and their families that are consistent with cultural, social, and physiological requirements.
Works with the diabetes educator/coordinator for each facility in coordinating services aimed at reducing the incidence of diabetes, assists in maintaining the diabetes registry assuring all data is current and accurate, analyzes and interprets diabetes data, compiles diabetes-related reports including diabetes audit and cost and care report.
Responsible for educating diabetic patients about disease, process, management, complications, and treatment using the Association of Diabetes Care and Education Specialists (ADCES).
Assist the diabetes educator/coordinator in coordinating all preventive care for the diabetic patients, including, but not limited to annual labs, EKG, retinal eye exams, dental exams, etc.
Observe patients, record significant conditions and reactions, and notify the supervisor or primary care provider (PCP) of the patient’s condition and reaction to drugs, treatments, and significant incidents of in-person visits.
Initiates a patient education plan according to the individualized needs of the patient, as prescribed by the PCP including patient and family instruction.
Participates in department or WRFCHC quality improvement activities as needed.
Keep up with or contribute to the latest food and nutritional science research.
Other duties as assigned to maintain/enhance the operation of the department.

QUALIFICATIONS:

Bachelor’s degree in dietetics, food, nutrition, food service management, institution management, or related science. The curriculum must have been in accordance with ACEND (Accreditation Council for Education in Nutrition and Dietetics)
Must be licensed by the Wyoming Dietetics Licensing Board
Knowledge of food service management, food sanitation and safety.
Knowledge of current federal regulations that govern programs with a nutrition component to provide consultation to Native American groups.
Must be culturally sensitive to the Native American population.
Knowledge and ability to provide nutrition therapy care to a specific age of the patients served.

Dialysis

DEPARTMENT: Dialysis
REPORTS TO: Nurse Manager

JOB SUMMARY:

Dialysis Nurses provide direct and individualized nursing care to patients based on the application of scientific nursing principles. Once on-the-job training is complete, in addition to general nursing care, responsibilities of Dialysis Nurses include (but are not limited to):

DUTIES AND RESPONSIBILITIES:

Conducts pre-dialysis patient assessment including obtaining patient vital signs and assessing the patient’s vascular access, laboratory findings, and the patient’s general health
Oversees Nephrology Technicians in the performance of dialysis from start to finish, monitors patient reaction to treatment, blood pressure and performance of the dialysis machines every 30 minutes and demonstrates an in-depth understanding of the mechanics of dialysis.
Reviews physician orders for dialysis patients, collects pre-treatment dialysis data, and reviews patient records prior to dialysis treatments.
Reviews physician orders from the Nephrologists as appropriate and administers dialysis related intra-venous and/or oral medications as prescribed and/or per protocol.
Verifies that patients are taking all prescribed medications and performs medication reconciliation.
Documents findings of patient’s assessments and interventions and advise Nephrologist of any significant change in the patient’s condition and other pertinent information
Performs other treatments and diagnostic tests such as blood sugar testing and other tests required as part of patient treatment.
Responds appropriately and provides assistance in emergency situations.
Continually functions as a patient educator. Provides patient education/teaching applicable to the patient’ s medical condition.
Conducts post dialysis assessment and patient discharge by evaluating treatment, documenting medication administration utilizing the appropriate protocols, assessing condition of vascular access, discharging the patient when vital signs are stable, and documenting and recording patient treatment data.
Discusses patient concerns and answers questions relevant to care.
Assesses, prepares, and cannulates a patient’ s vascular access (including needle insertion in arteriovenous fistula or grafts for aseptic connection with dialysis equipment).
Utilizes sterile techniques to assess the dialysis catheter exit site and to apply dressing on the patient’ s vascular access (dialysis catheter, arteriovenous access) per protocols.
Confirms acceptable water quality prior to beginning dialysis treatments. Cleans and disinfects dialysis equipment before and after each treatment.
Oversees disassembly and disinfection of dialysis equipment before and after each treatment. Disposes of biohazardous waste in accordance with policy & procedure
Maintains equipment per manufacturer’s recommendations. Labels and/or removes defective equipment from the treatment area and arranges for repair.
Performs surveillance and testing of dialysis water treatment equipment and dialysis water (such as chlorine, hardness, conductivity, water cultures) to ensure that these machines are operating within the parameters set in the facility’s written protocols.
Assures that machine alarms are functional prior to dialysis treatments.
Oversees the preparation of the delivery system, dialysate bath, and dialyzer and confirms that all mandatory alarm tests on the dialyzer and equipment are performed. Performs dialyzer reuse according to facility standards, if applicable.
Utilizes supplies efficiently to contain costs. Documents charges and services and/or credits supplies accurately.
Completes an incident report following any unusual occurrence that could have implications for risk management.
Cooperates with the End Stage Renal Disease Network 15 goal to collect data.
Refers patients to other disciplines or services as needed (Dietitian, Renal Social Worker, etc.).
Provides leadership in improving patient care, maintaining standards of care, directing and teaching personnel in a variety of settings, and providing charge nurse responsibilities as needed.
Participates in quality improvement activities to include collection of data for the dialysis unit
Quality Assurance Performance Improvement (QAPI) plan. Works closely with others on the dialysis team and participates in interdisciplinary team meetings.
Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

Associate degree in nursing with a current Registered Nurse license in the state of Wyoming.
Licensed as a registered nurse in the State of Wyoming.
One year of nursing experience in any field.
Must have Basic Life Support Certification.
Excellent verbal and written communication skills.
Must have knowledge through training or experience in performing patient care activities for adult and geriatric patients.
Required to possess a valid driver’s license and be insurable under the Tribe’s vehicle insurance plan.
Must be able to pass a criminal background check under the Indian Child Protection and Family Violence Act and the Wyoming DFS Central Registry.

Once on-the-job training is completed:

Must be able to perform surveillance on dialysis access (catheter, fistula, graft) for early signs of failure or infection; this includes (if needed and after receiving the appropriate training) the ability to perform arteriovenous access monitoring using ultrasound machine per protocol.
Must be able to do regional heparinization, separate and store blood samples, set up oxygen, administer saline, and maintain dialysis equipment.
Must demonstrate a working knowledge of dialysis laboratory tests and dialysis medications.
Must demonstrate in-depth understanding of infection control and prevention of transmission of blood borne pathogens.
Must be able to add, subtract, multiply, and divide all units of measure, percentages, and decimals.

The Tribal Epidemiologist is tasked with supporting data management, analysis, reporting, communication, and coordination of activities related to Wind River Family and Community Health Care (WRFCHC) epidemiology, public health practice, research and surveillance activities. The Tribal Epidemiologist will assist the community with identifying high-priority health status objectives and the services needed to achieve those objectives, based on epidemiological data.

Collect and analyze data to identify and assess community health issues and trends.

  Develop, monitor, and evaluate public health data policies for the community.

Conduct research to understand health risks and inequities posed for tribal members in the community.

Analyze existing or proposed systems, strategies, services and other health-related matters for the implementation and administration of the organizations public health programs and public-health activities.

 Participate in community and public information efforts.

Work collaboratively with tribal, inter-tribal, private, state and federal agencies to ensure that comprehensive, quality health data is available and accessible to the appropriate tribal entities.

 Design and utilize survey and mapping tools, data dictionaries, and relevant databases

Share information and collaborate with providers, families and other community resources to gain feedback on patient and community needs. 

Provide technical advice and assistance for data collection and analysis for the organization.

Identify medical, statistical, surveillance, public health, and research needs to plan and conduct projects.

Research connected health strategies to complement the current healthcare system   

Disseminate the result of investigations, studies, routine surveillance and analysis of health outcome data to the tribe.

 Provide leadership in planning appropriate investigations and interventions in epidemics and other adverse public health situations affecting the community.

 Participate in carrying out other priorities, plans, programmatic objectives, and policy formulations of WRFCHC, especially as related to health care planning and public health improvement.

QUALIFICATIONS:

      Must be a graduate of an accredited university or college; a PHD in Public Health is preferred.

The Human Resources (HR) Director is responsible for overseeing all aspects of human resources practices and processes. The Director will supervise employees of the Human Resources department to ensure all day-to-day transactional work is completed in a timely and efficient manner, and to provide back-up for these duties as necessary. JOB SUMMARY:

The Right-of-Way Coordinator performs all tasks related to tribal trust and fee right-of-way activities for the Northern Arapaho Tribe. The job responsibilities include performing all duties relating to trust and fee right-of-way activities, which includes subjects of deeds, leases, acquisitions, easements, mapping, surveying, legal descriptions, data management, document recording, planning, National Environmental Policy Act (NEPA), land use, zoning, and project management.

DUTIES AND RESPONSIBILITIES:

Work closely with Bureau of Indian Affairs (BIA) Realty Department on ROW process.
Work closely with all tribal departments that are in need of ROW transactions needed for current and future projects.
Act in leadership role for the Right-of-Way Department as needed.
Conduct property searches, title searches, appraisal reviews and recordation of land rights.
Read, review, and interpret complex legal descriptions, civil plans, and right-of-way maps.
Plan, manage and perform administrative duties consisting of, but not limited to, accurate data entry, preparing professional letters and email communications, with proper grammar, punctuation, spelling, and format.
Assist with preparation of annual operating budgets.
Correspond verbally and in writing with the public and internal/external customers on right-of-way related needs and attend meetings as needed.,
Perform any related right-of-way work and support as needed.
Learn the Geographic Information System (GIS) computer system.
Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

High school diploma required.
Associates Degree with 2 years of experience with real estate and GIS required.
Knowledge and ability to interpret legal descriptions, survey plats, tract maps, and legal records required.
Knowledge of computers and relevant software applications such as Word and Excel.

Assure that patients are seen in an orderly and fair manner, and that delays and waiting times are explained. Acutely ill and injured patients are triaged and seen first.

Initiate evaluation and care of the patient prior to the physician’s arrival according to standing orders. Consult with supervisors and doctors to decide on the best treatment plan for patients.

Prepare patients for examination and assist, as necessary. Arrange for follow-up care or referral.

Complete necessary forms and logs, documents medications and patient care in a timely and accurate manner.

Arrange for the transfer of patients to other departments or facilities (i.e. Emergency Dept.)

Be able to assist the provider on duty (MD, PA, or NP) with BLS, ACLS or PALS algorithms and to contact 911 to transfer patients to the Emergency Department.

Counsel and educate patients and their families on treatment plans.

Read and interpret patient information, making treatment decisions when appropriate.

Direct CMAs and CNAs as appropriate.

Handles disciplinary action for the Optometry staff in accordance with WRFCHC policy.

MCH Nurse develops, evaluates, and revises patient care goals in conjunction with patient and family.

Communicates with medical providers, family, and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner. 

Maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.

Builds strong working relationships internally with providers, nurses, First Impressions, and Benefit Coordinators to ensure patient has access to best possible healthcare. 

DUTIES AND RESPONSIBILITIES:

Carries out the Agency’s mission, philosophy, goals, and objectives within guidelines of Agency policy and position function.

A valid, current, full, and unrestricted Professional Nursing License is required.

A Bachelor of Science degree from an accredited School of Nursing preferred.

A minimum of 2-4 years’ experience working in a progressively more responsible nursing position.

Must demonstrate skills and proficiencies in Maternal Child Health, medical terminology, and practices.  

Must complete and maintain certification for lactation consultant and national passenger safety technician within the 60 days of employment.

JOB SUMMARY:

The purpose of this position is to perform a full range of Public Health Nurse duties in accordance with established Wind River Family and Community Health Care policies, procedures, and methods.

Assesses patient needs and then develops plans of care.  Consults with health care team members including participating in case management with health care providers.

Administers the therapeutic measures as prescribed by the health team, evaluates, and reassesses patient needs.

Shares health information and collaborates with patients/families and other community resources.  Provides health education to individuals, families, and community groups in various settings, i.e., schools, correctional facilities, community clinics, and other health-care settings.

Educates and coaches people to make appropriate and informed decisions about health issues.

Works with others to identify high-risk patient health problems and diagnoses.  Collaborates with other health care professionals to systematically track, provide services, and monitor these populations.

Performs segments of community assessments to develop a plan of action.  Conducts community analysis in partnership with community representatives, and in collaboration with other public health or health care officials and professionals.

Tracks community health status evaluates health trends and risk factors of population groups.  Collects data in a systematic and organized manner, i.e., demographic, immunization, and analyzes the assessment data.  Partners with community leaders to evaluate the data and determine opportunities and needs for the future.

Investigates health problems and hazards such as communicable diseases and develops a recommended plan of action to address these issues.

Consults, advises, and collaborates with individuals, families, communities, and local, state, and Federal systems.  Helps to determine the priorities for the targeted interventions.  Recommends interventions to mobilize resources for action.

Ensures that current epidemiological processes are followed.  Conducts epidemiological surveys, field investigations, and research studies.

Assess and determine needs working in collaboration with a variety of other organizations.  

Develop and participate in community health events and mass immunization clinics throughout the Northern Arapaho Community.  Settings will include schools, senior centers, community centers, and various other indoor and outdoor public locations within all our communities.

Collect data during screenings and immunization events; utilize this information to determine other potential community needs or public health issues that PHNs can participate in to help improve the health of the community.

Performs continuous evaluation of program and services, and performance improvement.  Develops Standards of Care and evaluates outcomes of interventions to continually improve population-based care.  

Other duties as assigned to maintain and enhance the operation of the department.

QUALIFICATIONS:

Must be a graduate of an accredited university or college; a Bachelor of Science in Nursing (BSN) is preferred however, an associate degree in nursing (ADN) may be accepted with the goal of obtaining BSN within 2 years of employment or less.

All nurses must be licensed as a Registered Nurse and provide evidence supporting certifications such as BLS and other necessary competencies (e.g., ACLS, TNCC, PALS, etc.).

Must have computer skills for the application of the EHR software.

Deep cleans each trailer after clients are discharged or as necessary. 

Verifies inventory levels and prevents inventory gaps for item such as Toilet paper, paper towels, cleaning supplies, etc. 

Provides appropriate oversight of the client’s stay and enforces rules of the shelter. 

Transports clients to and from the shelter for medical appointments as necessary

Writes and reports all incidents and maintenance issues to the Shelter Supervisor.

Submits Work Order Maintenance Requests as needed. 

Provides de-escalation techniques to clients as needed and to ensure safety.  

Monitors the clients, trailers, and grounds of the Shelter and ensure the safety and security of each client on the premise. 

Writes all observations in daily Turnover Notes.  

Must be able to conduct security searches in the event Security is not present. 

Must be able to work independently with little to no supervision. 

Performs other duties as assigned to maintain and enhance the operation of the Shelter.

QUALIFICATIONS:

High School diploma or GED.

Program Management:

Develop, implement, and evaluate the organizational infection prevention program.
Surveillance
Develop an annual surveillance plan based on the population(s) served, services provided, and analysis of surveillance data.

JOB OVERVIEW:

Provides services of safety and protection for all patients, visitors, staff, and company assets.

DUTIES AND RESPONSIBILITIES:

Conducts routine foot patrols or with an assigned vehicle to monitor for conditions or situations that may pose a risk to the safety of people or property. 

Provides escort services for visitors, patients, and staff. 

Maintains knowledge/follows policies and procedures regarding alarms and emergency/disaster preparedness plans. Facilitates planned drills and simulations.

Maintain security logs; Investigates and documents security incidents for 

Accurately receive and transmit information by radio, telephone, or computer system.

Maintains law and order by contact law enforcement in accordance with the jurisdiction. 

Informs and warns violators of rule infractions, such as loitering, smoking, or carry forbidden articles; responds to suspicious persons, vandalism and/or hazards.

Responds to clinical codes and emergencies including the de-escalation of an aggressive patient; implements emergency procedures when appropriate or as required.

Responds to first aid, cardiopulmonary resuscitation, and other emergency response procedure, as needed. 

Ensures all security and surveillance systems are working properly.

Locks and secures doors after hours according to schedule.

QUALIFICATIONS:

High School Diploma or GED.

Must possess a valid driver license.

One (1) year experience in security, law enforcement, or military, preferred but not required.

Must be able to obtain Basic Life Support within 60 days of hire.

Proficient computer skills in utilizing a variety of applications including Microsoft word.

JOB OVERVIEW:

The Family Advocate will work directly with victims of domestic violence, sexual assault, child abuse, mass violence and other types of victimization. 

DUTIES AND RESPONSIBILITIES:

Develop or maintain case plans for each family assigned.

Offer services and referrals from community partners, coordinating and accompanying victims and families to medical exams and court, attending any meetings the victims or family may request advocate support at and acting as a liaison between members of the MDT and child victims and their family.

Work with partner agencies to refer families to additional services as deemed appropriate, such as housing, healthcare, counseling, and educational resources. 

Ensure that all assigned individuals or families receive equitable services through case coordination, direct victim services, and by educating partner agencies on evidence based, best practices when working with families in crisis. 

Track and maintain all records of interactions and services provided by all involved agencies for each individual being assisted.

Communicate across systems to effectively coordinate services between WRFCHCS and entities serving the Wind River Reservation as pertaining to victims.

Work under stressful situations and must be highly motivated to ensure that the highest possible quality of care to all victims regardless of age, sex, or sexual identity.

Educate community members on the topic of child abuse and victimization to include courses, classes, training, and any other events when invited to speak on the topic with supervisor approval.

Complete 40 hours of Victim Assistance Training and 8 hours of additional training every 2 years.

General phone skills, office equipment, and computer skills are required. 

Other duties as assigned to maintain/enhance the operation of the Victim Services department.

QUALIFICATIONS:

Education: Bachelor’s Degree or equivalent training in the victim advocacy or social services field.  TVS will also take into consideration life experiences and/or years of experience.

Family advocates may be scheduled and stationed at the various WRFCHCS locations to increase accessibility for victims and decrease response time when a crisis occurs. Certified Addiction Practitioner (CAP) is a person certified to practice addictions treatment, prevention, intervention, diagnosis, referral and follow up under the supervision of a qualified clinical supervisor.

Completes Addiction Severity Index (ASI) assessment to determine appropriate ASAM Level of Care.

Develops individualized treatment plans with patients based on ASI assessment and ASAM Level of Care Recommendations.

Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.

Completes case management tasks as necessary.

Completes necessary documentation according to CARF accreditation and 3rd party payer standards.

Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.

Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures

Collaborates with WHRFCHC and other Tribal departments for continuity of care.

JOB OVERVIEW:

This position must foster a client’s rehabilitation from a diagnosed substance abuse disorder by organizing needed services and support into an integrated system of care until the client is able to assume this responsibility.

DUTIES AND RESPONSIBILITIES:

Assist and motivate clients to achieve and maintain basic life skills to be proactive in society.
Assist in the practice of addictions treatment, prevention, intervention, referral and follow up under the supervision of a qualified clinical supervisor.
Assess clients’ strengths and needs, utilizing screening tools that best fit their level of care.
Assist in organizing services and support an integrated system of care with other agencies.
Provides clients assistance in referring to services outside of the agency such as: inpatient facilities, menta] health, vocational rehab, etc.

Monitor client cases as they process through a comprehensive system of care.

Develops individualized treatment plans with patients based on ASI assessment and ASAM Level of Care Recommendations.
Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.
Completes case management tasks as necessary.
Completes necessary documentation according to CARF accreditation and 3rd party payer standards.
Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.
Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Assist and supervise clients with preparation of meals. Assist clients with scheduling appointments, chores, community service and other activities. Assist in the transportation of clients, adhering to all traffic and safety laws. Communicate difficulties and/or concerns regarding client’s behaviors to the supervisor as appropriate. Ensure shift chores are completed, especially the deep cleaning of the facility. Conduct alcohol and drug screening in conjunction with visits and employment. JOB OVERVIEW:

Provides assists in the oversight and management of the White Buffalo Recovery Transitional House.  This facility is an ASAM Level 3.1 Community Transitional Home operated in Arapahoe.

DUTIES AND RESPONSIBILITIES:

Works daily with residents to provide support, encouragement, and guidance.

Assists in coordination of crisis and emergency situations with the clinical staff.

Work in conjunction with other department staff to administer scheduled program curriculum and other activities utilized for treatment and rehabilitation of substance abuse.

Process new clients, i.e., issuance of clothing, assist with food resources, orientation to the rules of facilities and assists clients with daily activities, events and informational literature.

Assist the manager with daily operations of the facility including answering phones, processing time, maintaining adequate food supplies, and chore assignments.

Provide supervision and oversight of substance abuse technicians in absence of the manager, including all employee communication, training, mandatory meetings, and coverage.

Other duties as assigned to maintain/enhance the operation of the WBRC department.

QUALIFICATIONS:

GED/High School Diploma.

Associate degree in Addiction Studies or Human Services field.

Prefer experience is residential setting working with adults with substance abuse issues.

IMPORTANT APPLICATION CRITERIA:

Supporting documents:  Driver’s License, Tribal ID, High School Diploma/GED, Degree or professional credentials, and other supporting documents that verify required qualifications.

Applicants not living in the local area may submit a cover letter, resume, and supporting documents in lieu of an application as an initial statement of interest for a particular job announcement. However, the applicant must follow-up with an official Wind River Family and Community Health Care job application. 

Applicants who have a current application on file are required to submit a letter of interest for each position in which they want to be considered for. The letter should address how they meet each qualification. Supporting documents must be attached to the letter of interest.  

Telephone calls are not accepted in place of an employment application or letter of interest. 

When the position qualifications require a driver’s license, education, or professional credentials, the applicant must submit copies of certificates, licenses, degrees, and other supporting documentation. Applicants who do not provide supporting documents will be disqualified.
