JOB SUMMARY:

A Behavioral Health Clinician serves as a member of the Behavioral Health Team in the outpatient clinic at the Wind River Family & Community Health Care Center.  Providing behavioral health treatment to individuals, families, and groups.

DUTIES AND RESPONSIBILITIES:

Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations

Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff

Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care as required

Participates with other medical professionals in educating the community and medical staff on mental health issues

Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice

Collaborates with WHRFCHC and other Tribal departments for continuity of care

Other duties as assigned to maintain/enhance the operation of the Behavioral Health Department

QUALIFICATIONS:

Masters’ level education or higher in Social Work, Professional Counseling, Marriage and Family Therapy

Current licensure, or the ability to obtain the appropriate licensure within a reasonable amount of time (under the supervision of personnel currently in place) and has already passed the test required for licensure

Must possess a valid driver license and be eligible for coverage under WRFCHC auto insurance policy.

JOB OVERVIEW:

Psychologists will assist the Behavioral Health Department by providing both psychological evaluations and consulting to behavioral health clinicians as well as provide individual and group mental health therapy.

DUTIES AND RESPONSIBILITIES:

Responds to referrals from the Behavioral Health and Medical Providers
Provide competent and prompt psychological evaluations to children, adolescent, and adult patients to facilitate the identifying of needed resources and determining treatment recommendations.
Provide psychological testing within their scope of practice and completes reports in timely manner.
Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations.

As a team member develop productive and cooperative working relationships with health care providers within the facility as well as allied healthcare and social service providers.
Performs other duties as assigned to maintain and enhance the operation of the program.

QUALIFICATIONS:

PhD in psychology or PsyD with current independent licensure.
Must be committed to providing comprehensive health care to the individual, family, and community.
Must maintain strict confidentiality and high ethical standards in performing the position.

JOB SUMMARY:

The Benefits Coordinator helps Wind River Family and Community Health Care (WRFCHC) patients navigate possible alternate resources, (health insurance, benefits, etc.) ensuring they receive appropriate care and coverage. They assist with eligibility, applications, and enrollment in various programs including Medicaid, Medicare, clinic purchased insurances, etc. This role also involves providing alternate resource education, referrals, and support to patients and their families.    

DUTIES AND RESPONSIBILITIES:

Acts as an advocate for WRFCHC patients in securing alternate resources such as Medicare, Medicaid, Department of Veterans Affairs, State Programs, Third Party Liability, and Children’s Special Health Services.

Identifies patients who may be eligible for alternate resources through the effective utilization of pre-screening and interviewing methods.

Understand and recognize what programs each patient/family would be eligible for based on their unique circumstances.

Works closely with all WRFCHC departments in the identification and utilization of all alternate resources available to the patient population.

Helps patients complete applications for benefits and assists with enrollment processes.

Provides information and resources to patients about their benefits, coverage options, etc.

Connects patients with appropriate resources and support services.

Assists patients with eligibility renewals.

Maintains open communication with patients, families, and healthcare providers to ensure coordinated care.

Maintains accurate records of patient information, benefits, and eligibility status.

Ensures compliance with all applicable regulations and guidelines related to benefits administration.

Addresses any issues or concerns that may arise with patient benefits.

Advocates for patients and families for their needs within the complex healthcare systems.

Interviews patients using established criteria.

Stay current on different programs’ regulations and policies.

Respond to referrals from patient registration, Purchased Referred Care (PRC) and other departments/committees as needed.

Educates different WRFCHC departments on updates to programs (private insurance, Medicaid, Medicare, etc.)

Provides outreach, education, and assistance regarding alternate resource programs to patients and community members.

Timely alternate resource application follow up.

Notify pharmacy, business office, and PRC of approved alternate resources with effective date of coverage.

Prepare reports for supervisor and/or administration.

Documents patient eligibility, alternate resource approval/denial, screenings, application status in appropriate Electronic Health Record and/or spreadsheets.

Partner with local and tribal programs to provide outreach and education.

Works closely with the Social Security Administration on local and regional levels.

Works closely with the Tribal TANF and DFS to establish eligibility for alternate resources.

Works with Social Services and other entities that may assist in the development of registry of eligible patients.

Must be willing to travel to a patient’s residence for assistance in application completion.

Must be willing to become a Certified Application Counselor through ACA Marketplace.

QUALIFICATIONS:

High School Diploma or equivalent required.

An associate degree in health care, business or a related field preferred but not required.

Certification through CMS (Center for Medicare and Medicaid Services) preferred.

One year of medical office experience is preferred.

Experience and knowledge of third-party payers, statutes and regulations that impact eligibility for health insurance coverage.

Familiarity and understanding of health insurance.

Ability to be highly organized, display excellent time management skills, and be proactive.

Experience with Electronic Health Records (EHR) is preferred.

JOB SUMMARY:

This position involves rotating at different facilities as needed to ensure infection prevention across all WRFCHC facilities. 

DUTIES AND RESPONSIBILITIES:

Perform all assigned tasks in a professional manner to reflect the highest integrity of the department to ensure the continuity and efficiency of patient care and daily operations.

Perform routine cleaning of exam rooms, nursing stations, offices, clinical areas, waiting areas, lobbies, lounges, rest rooms, corridors, and stairways in a clean, neat, and sanitary manner.

Perform routine cleaning of high traffic areas of walls, cabinet surfaces, tables, stands and any other furniture and areas as directed.

Perform cleaning and maintenance of all floors: carpet, tile, brick, and concrete flooring.

Completes trash, biohazardous linens, and sharps container removal to appropriate disposal area.

Maintain inventory of assigned areas cleaning supplies, paper products, and other essential items required to keep all areas functioning.

Utilizes the appropriate chemicals, supplies, and cleaning techniques according to procedure and manufacturers’ recommendation.

Assists visitors with directions whenever requested.

Is trained/cross trained in all facilities to ensure ability to meet workload requirements when staffing shortages and/or vacations reduce staff levels.

Report safety hazards as appropriate.

Replenishes supplies; notifies supervisor or designee when supplies need to be replenished.

Follows all custodial services practices including hand washing techniques, standard precautions, and isolation precautions.

Observe and follow all health and safety regulations at all times.

JOB SUMMARY:

The Dental Director position oversees and manages the operations of Wind River Family and Community Health Care’s dental practice focusing on patient care and staff management ensuring compliance and quality standards are met. This position requires leadership, coaching, organizational, communication, analytical, and problem solving skills.

DUTIES AND RESPONSIBILITIES:

Supervisory:

Develops and writes the policy and procedures and standing operating basis for the Dental  Department in alignment with WRFCHC guidelines.

Participates in organizational meetings to develop new, modify existing and evaluate medical procedures relative to patient care with the objective of constantly improving and increasing the degree of patient care given to clients.

Maintain scheduling and timesheets of Dental staff  and set policy and standards for the department.

Oversees the daily workflow of the Dental department.

Complete Performance Evaluations and corrective action plans (when necessary) for those supervised.

Handles disciplinary action for the Dental staff in accordance with WRFCHC policy.

Make appropriate referrals to other specialists or services as needed.

Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice.

Review monthly billing totals and find opportunities to help increase collections.

Maintains a staff of sufficient numbers to effectively provide quality care for patients of all ages.

Clinical:

Makes clinical and radiographic examinations of the oral hard and soft tissues and a diagnosis of pathological or irregular conditions of the oral cavity and maxillofacial area.

Performs endodontic, prosthodontic, periodontic, operative, preventative, and restorative dentistry within the scope of practice and ability.

Performs tooth extractions and refers as needed to the oral surgeon.

Performs root canal therapy within scope of practice.

Treats scheduled and emergent patients as needed.

Ensures accurate input of patient demographics into Dentrix Database.

Informs patients about good dental health practices.

To maintain a sterile/disinfected environment in the delivery of oral care. 

QUALIFICATIONS:

Applicants must have a Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) degree from a school approved by the American Dental Association (ADA) or the Commission on Dental Accreditation (CODA) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Applicants must have passed the National Board Dental Examinations (NBDE) which are developed and administered by the Joint Commission on National Dental Examinations (JCNDE) and possess a current, full, and unrestricted license to practice dentistry in the State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Two years of supervisory experience preferred.

JOB SUMMARY:

On-the-job training provided. The patient care technician provides safe and acceptable dialysis under the supervision of a registered nurse. Communicates with patients, families, and the health care team to provide services according to the patient care plan.

DUTIES AND RESPONSIBILITIES:

Performs basic patient care activities.

Observes patients and reports unusual conditions and/or reactions to RN.

Obtain blood samples for laboratory analysis as required to complete physician orders.

Perform duties at all times under the supervision of a Registered Nurse and within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations. 

Use appropriate safety measures, including personal protective equipment as necessary.

Clean, disinfect, and rinse dialysis machines in accordance with policy and procedures.

Prepare machines for hemodialysis treatments. 

Obtain pre and post treatment weights of patients per protocol. 

Safely initiate and terminate dialysis treatments per center protocol, and within scope of practice and as allowed by state regulations. 

Monitor patients during dialysis treatments. 

Mix acids and bicarbonate according to protocol. 

Complete water testing on a dialysis machines at the beginning of each treatment. Complete and document quality control logs throughout the day and after all treatments, as required.

Responds to the patient’s needs with compassion and concern.  

Participate in staff meetings as required, including in-service and continuing education that are offered for certifications.

JOB SUMMARY:

Seeking a primary care provider to provide compassionate, patient-centered, and culturally responsive care to our medically complex and underserved population. Serving as the primary point of access to healthcare, the PCP delivers comprehensive services including preventive care, diagnosis, and management of acute and chronic conditions. This role emphasizes addressing health disparities, improving access to care, and supporting patients who may face socioeconomic, linguistic, or systemic barriers.

The PCP collaborates with an interdisciplinary team to coordinate care, connect patients with community resources, and promote health equity. Responsibilities include managing complex medical and social needs, engaging in patient education, and contributing to population health initiatives aimed at improving outcomes in vulnerable communities.

DUTIES AND RESPONSIBILITIES:

Provide comprehensive primary care services, including preventive, diagnostic, and therapeutic care.

Perform physical examinations and obtain medical histories.

Diagnose and treat acute and chronic medical conditions.

Order, interpret, and evaluate diagnostic tests (labs, imaging, etc.).

Prescribe medications and develop treatment plans.

Manage chronic diseases such as diabetes, hypertension, asthma, and heart disease.

Provide health maintenance screenings and immunizations.

Offer counseling and education on wellness, disease prevention, and lifestyle modifications.

Coordinate referrals to specialists and follow up on consultative services.

Maintain accurate, timely, and complete documentation in the electronic health record (EHR).

Participate in quality improvement initiatives and team-based care meetings.

Comply with all applicable federal, state, and organizational regulations.

QUALIFICATIONS:

Doctor of Medicine (MD),  Doctor of Osteopathic Medicine (DO), Master of Nurse Practitioner (NP), or Master of Physician Assistant (PA) 

Permanent, current, full, and unrestricted license in any state in the United States, Territory, or District of Columbia. 

If a physician, completion of an ACGME-accredited residency program in an appropriate specialty such as Family Medicine, Internal Medicine/Pediatrics, or Pediatrics or board eligibility is required. 

Current, full, and unrestricted DEA license. 

Current BLS, ACLS, and PALS certifications or be able to obtain them within 60 days of hire.

Must possess a valid driver’s license and be eligible for coverage under the WRFCHC auto insurance policy. 

Must be able to pass a criminal background check under the Indian Child Protection and Family Violence Prevention Act (P.L.101-630).

WORKING ENVIRONMENT:

The provider will see, on average, 14 patients per clinic workday. 

The provider schedule is four 10-hour shifts or five 8-hour shifts per week, Monday through Friday.

The provider will be based in our outpatient clinics. The provider will typically have one CNA and one MA on their team, as well as a shared RN and 4 or 5 provider colleagues working at the same location. Providers may also be expected  to provide care at local nursing homes, schools, children’s homes, homeless shelters or other community based facilities. We try to align providers’ interests and areas of expertise when matching providers to these community needs. 

If the provider is interested they may be able to join our in patient care group which rotates call at our local hospital. Average 3-5 24hr shifts a month. Must be a physician, the local hospital does not credential NP/PA providers. 

The provider will be a part of a rotation pool to staff weekend and evening shifts at our clinic in Riverton. The provider will not be scheduled for more than 6 of these shifts a year. They will be expected to serve as ‘back up call’ for this coverage no more than 4 weekends a year. These shifts are from 12:30-10:30pm Friday through Monday. Details about inpatient hospital duties can be discussed in detail if indicated. If a provider takes on in patient care they are exempt from these expectations.  

Primarily works in a clinical environment, including outpatient exam rooms, treatment areas, and administrative office settings with potential exposure to infectious diseases, bodily fluids, hazardous materials, and sharp instruments. 

The environment can be fast-paced with frequent interruptions, requiring the ability to prioritize tasks, make decisions quickly, and respond effectively to urgent medical needs.

May involve long periods of standing, walking, or sitting, as well as occasional lifting, bending, reaching, and crouching.

The MCH Nurse will promote maternal self-care and infant care, assess and monitor maternal physical and emotional changes, foster parent infant relationship, assess and advise on safety of infant environment.  Provide assessment and teaching and foster independence with breastfeeding and monitor infant growth and development.  MCH Nurse develops, evaluates and revises patient care goals in conjunction with patient and family.

QUALIFICATIONS:

A valid, current, full and unrestricted Professional Nursing License is required. A Bachelors of Science degree from an accredited School of Nursing preferred, and  minimum of 2-4 years of experience working a progressively more responsible nursing position.

JOB SUMMARY:

Telemedicine uses videoconferencing equipment to send and receive medical information and pictures. The Telemedicine Nurse facilitates this technology to serve patients who require specialized care in a distant community.

The Telemedicine Nurse will serve as a clinical resource and help to develop, implement, evaluate and design Telemedicine workflows and measures to ensure optimal integration into delivery of care and service. 

DUTIES AND RESPONSIBILITIES:

Demonstrates competency in knowledge and skills of Telemedicine Nurse and site coordination roles.

Informs and updates providers and staff of services available through Telemedicine.

Provides specialist site with any necessary information.

Follows the referral process/coordinates appointments with specialist, patient, and clinical site.

Facilitates equipment set-up and indicated quality control activities in preparation for appointment.

Provides follow-up:

Sees that requested lab/tests are ordered and results are sent to specialist.

Schedules follow-up appointments, ensures referrals are sent.

Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.

JOB SUMMARY:

Coordinates and provides comprehensive medication management to optimize medication use.

Documents all clinical activities, updates problem lists and records information in the electronic health record when required and maintains pharmacy records as required by law.

 Provides ongoing, comprehensive chronic disease management.

Perform departmental administrative functions, committee involvement, quality assurance activities, statistical analysis, CME presentations, in-services, competency and proficiency reviews and involvement in performance improvement groups. Responds to inquiries and requests from staff at WRFCHC such as medical, social services, dental, etc. 

Responsible for implementation of decisions made for provision or denial of services. 

Works as part of a team in planning, coordination, and management of the health care delivery system.

 Assures adequate funds are set aside for all authorized referrals, emergency call-ins, and follow up care referrals. 

Assures all funds have been obligated by the end of the funding period and that all unused obligations are canceled on a timely basis. 

Assures all outstanding authorizations over 90 days old are valid; others are canceled and funds are obligated before the end of the fiscal year. 

Assures authorizations are processed when funds are available. 

Assures transactions affecting funds are promptly processed. This includes obligations, disbursements, cancellations, and recording allocations. 

Assures effective communications with Fiscal Intermediaries and Area Finance. 

Responds to all pending claims on a  timely basis.

JOB SUMMARY:

The Right of Way Coordinator performs all tasks related to tribal trust and fee right-of-way activities for the Northern Arapaho Tribe through WRFCHC.   The job responsibilities include performing all duties relating to trust and fee right-of-way activities, which including deeds, leases, acquisitions, easements, mapping, surveying, legal descriptions, data management, document recording, planning, National Environmental Policy Act (NEPA) assessment, land use, zoning, and project management. DUTIES AND RESPONSIBILITIES:

Work closely with the Bureau of Indian Affairs (BIA) Realty Department on the ROW processes.

Work closely with all tribal departments that are in need of ROW transactions needed for current and future projects.

Lead the daily responsibilities of the ROW Department as needed. 

Conduct property searches, title searches, appraisal reviews and recordation of land rights.

Read, review, and interpret complex legal descriptions, civil plans, and right-of-way maps.

Plan, manage and perform administrative duties consisting of, but not limited to, accurate data entry, preparing professional letters and email communications, with proper grammar, punctuation, spelling, and format.

Provide direction and supervision throughout the NEPA process for all IHS projects. Direction includes scoping, identifying issues, developing solutions and alternatives, and fulfilling NEPA assessment documentation. 

Work closely with Sanitation Facilities Construction (SFC) Department to complete and submit National Environmental Policy Act (NEPA) documents.  

Assist with preparation of annual NATED operating budgets.

Correspond verbally and in writing with the public and internal/external customers on right-of-way related needs and attend meetings as needed.,

Perform any related right-of-way work and support as needed.

Learn the Geographic Information System (GIS) computer system.

Associates Degree with 2 years of experience with real estate and GIS required.

Dig trenches during excavation operations using a shovel, rake and various digging tools.

Assemble and disassemble construction equipment like scaffold, ramps and braces.

While performing duties of this job, the employee will be exposed to moving mechanical parts and outside weather conditions.

Certified Addiction Practitioner (CAP) is a person certified to practice addictions treatment, prevention, intervention, diagnosis, referral and follow up under the supervision of a qualified clinical supervisor.

Completes Addiction Severity Index (ASI) assessment to determine appropriate ASAM Level of Care.

Develops individualized treatment plans with patients based on ASI assessment and ASAM Level of Care Recommendations.

Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.

Completes case management tasks as necessary.

Completes necessary documentation according to CARF accreditation and 3rd party payer standards.

Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.

Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures

Collaborates with WHRFCHC and other Tribal departments for continuity of care.

Responsible for the supervision of the Transitional Home during the weekend.  The Weekend Supervisor will provide a safe, trauma informed, supportive environment for all residents.

DUTIES AND RESPONSIBILITIES:

Supervise staff and maintain a schedule for all weekend employees.

Treat all clients with dignity and respect and work with residents to provide support, encouragement, and guidance.

Accurately report incidents or any behavior that includes significance on provided forms.

Assist and supervise clients with preparation of meals.

Assist clients with scheduling appointments, chores, community service and other activities.

Assist in the transportation of clients, adhering to all traffic and safety laws.

Communicate difficulties and or concerns regarding client’s behaviors to the supervisor as appropriate.

Ensure shift chores are completed, especially the deep cleaning of the facility.

JOB SUMMARY:

The Licensed Substance Abuse Provider provides professional services to individuals and groups with substance use disorders; these persons may practice independent of supervision, unless provisionally licensed, in which case they will be assigned a clinical supervisor.  This position may be located at either Riverton, Arapahoe or Ethete offices.

DUTIES AND RESPONSIBILITIES:

Provides services based on scope of practice of Licensure including, but not limited to, addition     prevention, crisis intervention, diagnosis, referral, direct treatment, and follow-up treatment which is rendered to individuals, families, groups and/or organizations who are adversely affected by addictions or related disorders. 

Interviews clients and families to complete an assessment tool such as Addiction Severity Index (ASI) and will develop an individualized treatment plan

Applies therapies and interventions to address patients’ cognitive and behavioral impairment by counseling clients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life and making changes. . 

Clinical supervision of Certified Addiction Practitioner (CAP) and Certified Addiction Practitioner Assistants (CAPA), acting as a mentor, if Licensed;

 Provide coverage for Clinical Coordinator when needed. 

Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.

Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures.

Stive to maintain client centered practices by integrating family and positive support systems by maintaining professional standards of conduct, and model recovery type behavior.

Completed a graduate degree program with concentration on addictionology, chemical dependency, or substance abuse from an educational institution accredited by one of the regional or national institutional accrediting bodies recognized by the Council for Higher Education Accreditation (CHEA)

DUTIES AND RESPONSIBILITIES:

Provides assistance and technical support to the community about substance use disorder prevention and education activities by conducting educational sessions on substance abuse, alcohol abuse, commercial tobacco and other drug related problems to local schools and community agencies/programs.

Support local youth groups in the community and schools through social/prevention activities such as but not limited:

To facilitate youth talking circles and support groups.

Cultural events and activities, experiential learning activities, leadership, and youth empowerment events.

Promoting overall wellness and supporting wellness activities in the community.

Create and nurture protective factors (such as caring relationships, high expectations, and relevant and meaningful participation) in each youth’s environment, including but not limited to, establishing relationships that encourage positive social and emotional development, and integrating and reconnecting youth with natural supports, community, and cultural resources.

Facilitate youth engagement and self-determination, such as engaging youth to build and strengthen their identity and role in the Native community and facilitating interdepartmental activities to engage youth.

Develop prevention community resources utilizing media, social media, advising, fact sheets, etc. Support the Youth Media Center and assist in the development of prevention videos and films.
