Serves as a member of the Behavioral Health Team in the outpatient clinic at the Wind River Family & Community Health Care Center. Providing behavioral health treatment to individuals, families, and groups.
Provides intakes, assessments, and treatment planning for patient care.

Provides mental health case management to patient of the behavioral health departments. Case manager will follow up, monitor, participate in crisis management, and connect with other services.
Provides case management services consisting of:
Linkage – Working with clients and/or service providers to secure access to needed services. Activities include communication with agencies to arrange for appointments or services following the initial referral process and preparing clients for these appointments. Contact with hospitalized clients, hospital/institution staff, and/or collateral to facilitate the client’s reintegration into the community.
Monitoring/Follow-Up – Contacting the client or others to ensure that a client is following a prescribed service plan and monitoring the progress and impact of that plan.
Referral – Arranging initial appointments for clients with service providers or informing clients of services available, addresses, and telephone numbers of agencies providing services.
Advocacy – Advocacy on behalf of a specific client for the purpose of accessing needed services. Activities may include making and receiving telephone calls and the completion of forms, applications, and reports which assist the client in accessing needed services.
Crisis Intervention – Crisis intervention and stabilization are provided in situations requiring immediate attention/resolution for a specific client. The case manager may provide the initial intervention in a crisis and would assist the client in gaining access to other needed crisis services.
Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, up to date clinical records, including progress notes and other relevant information, and enter the necessary information into the electronic health record system.

Functions as a member of the clinical team, assessing nutritional needs of WRFCHC patients. This includes, but is not limited to planning, coordinating, and evaluating the nutritional component of health care services for specific comorbidities and throughout the life span.
Works independently and in collaboration with WRFCHC medical team(s) and community outreach programs.
Conducts nutritional assessment and provides Medical Nutrition Therapy (MNT) based on disease process, using the Nutrition Care Process (NCP). Disease processes may include but are not limited to prediabetes, diabetes, gestational diabetes, diabetes during pregnancy, adult obesity, childhood obesity, malnutrition, chronic kidney disease, hypertension, and hyperlipidemia.
Provides diabetes nutrition counseling/education to the patients and their families that are consistent with cultural, social, and physiological requirements.
Works with the diabetes educator/coordinator for each facility in coordinating services aimed at reducing the incidence of diabetes, assists in maintaining the diabetes registry assuring all data is current and accurate, analyzes and interprets diabetes data, compiles diabetes-related reports including diabetes audit and cost and care report.
Responsible for educating diabetic patients about disease, process, management, complications, and treatment using the Association of Diabetes Care and Education Specialists (ADCES).
Assist the diabetes educator/coordinator in coordinating all preventive care for the diabetic patients, including, but not limited to annual labs, EKG, retinal eye exams, dental exams, etc.
Observe patients, record significant conditions and reactions, and notify the supervisor or primary care provider (PCP) of the patient’s condition and reaction to drugs, treatments, and significant incidents of in-person visits.
Initiates a patient education plan according to the individualized needs of the patient, as prescribed by the PCP including patient and family instruction.
Participates in department or WRFCHC quality improvement activities as needed.
Keep up with or contribute to the latest food and nutritional science research.
Other duties as assigned to maintain/enhance the operation of the department.
Bachelor’s degree in dietetics, food, nutrition, food service management, institution management, or related science. The curriculum must have been in accordance with ACEND (Accreditation Council for Education in Nutrition and Dietetics)

Knowledge of current federal regulations that govern programs with a nutrition component to provide consultation to Native American groups.

The Safety/OSHA Officer is responsible for developing, implementing and monitoring the safety program and initiatives for the organization. This position is responsible for minimizing hazards and reducing risk of injury to patients, employees, visitors, and vendors. This position coordinates the on-going, organization-wide system of information collection and evaluation in order to identify deficiencies and opportunities for improvement for accreditation standards.
Develops, implements and monitors the patient and employee safety program.
Evaluates and improves organizational patient and employee safety based on internal needs and external requirements and standards.
Directs the development of a comprehensive Patient and Employee Safety Plan and updates the plan as needed.
Works with administrative/management staff to develop and implement systems and procedures for collection of information.
Develops systems for and coordinates the on-going collection and response to various sources of information that impact the organization, such as published hazard notices or recall reports.
Prepares findings, recommendations, actions taken, and disseminates results of performance improvement activities.
Documents situations and trends and plans for remediation of situations that present risk, including such matters at the disposal and tracking of hazardous waste.
Works with clinical and administrative staff to develop and implement safety policies and procedures.
Prepares and submits reports concerning findings, recommendations, actions, and monitoring activities to the Compliance Officer on a bi-weekly basis or as requested by the Compliance and Risk Management committees.
Assists Risk Management in conducting physical surveys of facilities and compliance with OSHA recommendations;
Actively participates in safety management and planning;
Ensures the development of surveillance systems to identify and mitigate potential patient/employee harm;
Works with administrative/clinical supervisors to ensure that the objectives, scope, performance, and effectiveness of management plans are evaluated at least annually based on the analysis of incident reports and other information;
Coordinates organizational response to regulatory standards related to patient/employee safety;
Assists in defining performance indicators for evaluation of the safety program and in reporting performance indicators of all areas to the Compliance and Risk Management committees; and

Perform all assigned tasks in a professional manner to reflect the highest integrity of the department to ensure the continuity and efficiency of patient care and daily operations.
Perform routine cleaning of exam rooms, nursing stations, offices, clinical areas, waiting areas, lobbies, lounges, rest rooms, corridors, and stairways in a clean, neat, and sanitary manner.
Perform routine cleaning of high traffic areas of walls, cabinet surfaces, tables, stands and any other furniture and areas as directed.
Perform cleaning and maintenance of all floors: carpet, tile, brick, and concrete flooring.
Completes trash, biohazardous linens, and sharps container removal to appropriate disposal area.
Maintain inventory of assigned areas cleaning supplies, paper products, and other essential items required to keep all areas functioning.
Utilizes the appropriate chemicals, supplies, and cleaning techniques according to procedure and manufacturers’ recommendation.
Assists visitors with directions whenever requested.
Is trained/cross trained in all facilities to ensure ability to meet workload requirements when staffing shortages and/or vacations reduce staff levels.

The Dialysis Patient Care Technician Position provides safe and acceptable dialysis treatment under the supervision of a registered nurse. Performs basic patient care activities.

Mix acids and bicarbonate according to protocol.
Complete water testing on a dialysis machine at the beginning of each treatment day and as directed by the Administrator. Complete/document quality control logs throughout the day and after all treatments, as required.

The Dialysis Registered Nurse position provides direct patient care to individuals undergoing dialysis treatment. Conducts pre-dialysis patient assessment including obtaining patient vital signs and assessing the patient’s vascular access, laboratory findings, and the patient’s general health
Oversees Nephrology Technicians in the performance of dialysis from start to finish, monitors patient reaction to treatment, blood pressure and performance of the dialysis machines every 30 minutes and demonstrates an in-depth understanding of the mechanics of dialysis.
Reviews physician orders for dialysis patients, collects pre-treatment dialysis data, and reviews patient records prior to dialysis treatments.
Reviews physician orders from the Nephrologists as appropriate and administers dialysis related intra-venous and/or oral medications as prescribed and/or per protocol.
Verifies that patients are taking all prescribed medications and performs medication reconciliation.
Documents findings of patient’s assessments and interventions and advise Nephrologist of any significant change in the patient’s condition and other pertinent informationc
Performs other treatments and diagnostic tests such as blood sugar testing and other tests required as part of patient treatment.
Responds appropriately and provides assistance in emergency situations.
Continually functions as a patient educator. Provides patient education/teaching applicable to the patient’ s medical condition.
Conducts post dialysis assessment and patient discharge by evaluating treatment, documenting medication administration utilizing the appropriate protocols, assessing condition of vascular access, discharging the patient when vital signs are stable, and documenting and recording patient treatment data.
Discusses patient concerns and answers questions relevant to care.
Assesses, prepares, and cannulates a patient’ s vascular access (including needle insertion in arteriovenous fistula or grafts for aseptic connection with dialysis equipment).
Utilizes sterile techniques to assess the dialysis catheter exit site and to apply dressing on the patient’ s vascular access (dialysis catheter, arteriovenous access) per protocols.
Confirms acceptable water quality prior to beginning dialysis treatments. Cleans and disinfects dialysis equipment before and after each treatment.
Oversees disassembly and disinfection of dialysis equipment before and after each treatment. Disposes of biohazardous waste in accordance with policy & procedure
Maintains equipment per manufacturer’s recommendations. Labels and/or removes defective equipment from the treatment area and arranges for repair.
Performs surveillance and testing of dialysis water treatment equipment and dialysis water (such as chlorine, hardness, conductivity, water cultures) to ensure that these machines are operating within the parameters set in the facility’s written protocols.
Assures that machine alarms are functional prior to dialysis treatments.
Oversees the preparation of the delivery system, dialysate bath, and dialyzer and confirms that all mandatory alarm tests on the dialyzer and equipment are performed. Performs dialyzer reuse according to facility standards, if applicable.
Utilizes supplies efficiently to contain costs. Documents charges and services and/or credits supplies accurately.
Completes an incident report following any unusual occurrence that could have implications for risk management.
Cooperates with the End Stage Renal Disease Network 15 goal to collect data.
Refers patients to other disciplines or services as needed (Dietitian, Renal Social Worker, etc.).
Provides leadership in improving patient care, maintaining standards of care, directing and teaching personnel in a variety of settings, and providing charge nurse responsibilities as needed.

Under the collaboration of the Chief Medical Officer, the Same Day Medical Provider’s primary responsibility is to provide same-day care to patients of all ages presenting on the walk-in cue. Patient concerns range from medication refill requests to urgent needs related to injury and acute illness. The Environmental Health Specialist (EHS) coordinates a standard approach to local environmental health issues surrounding the Northern Arapaho Communities. This position coordinates with Billings Area Office Personnel as well as local agencies to conduct and complete comprehensive surveys on local facilities, injury prevention, epidemiological investigations and reports all conclusions and findings to the appropriate agencies.
The EHS will submit monthly reports that summarize all environmental health service’s activities completed during that time frame to the supervisor and Chief Executive Officer of Wind River Family and Community Health Care (WRFCHC).
The EHS will complete survey reports on all facilities listed in the Indian Health Service (IHS) Environmental Health Reporting System, also known as WebEHRS, in accordance with current IHS Guidelines.
The EHS will educate communities on environmental health issues.
The EHS will perform a variety of functions in the environmental health field including providing technical assistance, conducting inspections or surveys, and responding to illness outbreaks and food recalls.
Plans, implements and evaluates a comprehensive environmental health program on the Wind River Reservation that emphasizes safe water, solid and hazardous waste management, food protection, occupational health and safety, vector control, community injury prevention, healthy homes, and institutional health.
Builds tribal capacity to manage environmental health hazards in their community by assisting with the development of environmental health and occupational safety policies, procedures, regulations, employee training programs and public education materials.
Serves as a technical consultant to tribal program managers and leaders by assisting them with environmental health hazard identification and mitigation.
Performs epidemiological and infection control investigations to identify environmental factors with the potential for transmission of disease.
Serves as the Community Injury Prevention facilitator for the Wind River Reservation by assisting the Northern Arapaho Tribe in the development of an interdisciplinary and epidemiological approach to reduce injuries.
Under the guidance of medical staff, advises the public on preventive health and safety precautions to follow during a potential environmental health threat.
Manage the WRFCHC integrated pest management approach for reducing populations of disease vectors and public health pests.
The EHS will provide oversight to the WRFCHC animal control operation to minimize zoonotic disease, animal bites and community education to vaccinate animals and spay/neuter pets.
Completes probationary and yearly evaluations for animal control staff in accordance with WRFCHC policy.

The Family Advocate will work directly with victims of domestic violence, sexual assault, child abuse, mass violence and other types of victimization.
Develop or maintain case plans for each family assigned.
Offer services and referrals from community partners, coordinating and accompanying victims and families to medical exams and court, attending any meetings the victims or family may request advocate support at and acting as a liaison between members of the MDT and child victims and their family.
Work with partner agencies to refer families to additional services as deemed appropriate, such as housing, healthcare, counseling, and educational resources.
Ensure that all assigned individuals or families receive equitable services through case coordination, direct victim services, and by educating partner agencies on evidence based, best practices when working with families in crisis.
Track and maintain all records of interactions and services provided by all involved agencies for each individual being assisted.
Communicate across systems to effectively coordinate services between WRFCHCS and entities serving the Wind River Reservation as pertaining to victims.
Work under stressful situations and must be highly motivated to ensure that the highest possible quality of care to all victims regardless of age, sex, or sexual identity.
Educate community members on the topic of child abuse and victimization to include courses, classes, training, and any other events when invited to speak on the topic with supervisor approval.
Complete 40 hours of Victim Assistance Training and 8 hours of additional training every 2 years.

This position must foster a client’s rehabilitation from a diagnosed substance abuse disorder by organizing needed services and support into an integrated system of care until the client is able to assume this responsibility.
Assist and motivate clients to achieve and maintain basic life skills to be proactive in society.
Assist in the practice of addictions treatment, prevention, intervention, referral and follow up under the supervision of a qualified clinical supervisor.
Assess clients’ strengths and needs, utilizing screening tools that best fit their level of care.
Assist in organizing services and support an integrated system of care with other agencies.
Provides clients assistance in referring to services outside of the agency such as: inpatient facilities, menta] health, vocational rehab, etc.
Explore and advocate for community-based services dependent on the clients’ needs and refer to non-substance abuse services.
Monitor client cases as they process through a comprehensive system of care.
Work with clients’ families to assist in creating a supportive environment for recovery.
Provide transportation to and from residential treatment centers in the State and surrounding areas.
Assist or facilitate outpatient education sessions.
Completes necessary documentation according to CARF accreditation and 3rd party payer standards.
Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.
Administer random substance use screening, to support continued sobriety.
Assist Certified Addiction Practitioner or Licensed Providers with the processing of Addiction Severity Index (ASI) and provide input and recommendations for treatment planning.
Maintain professional standards of conduct, and model recovery type behavior. Adhere to NAADAC Code of Ethics, as well as policies and procedures.

Minimum of an associate degree in human services, addictionology, psychology, or a related field.
Complete 270 hours of education and/or training with a minimum 200 hours specific to addictions, six hours of communicable diseases coursework, and at least 30 hours of counseling ethics. Up to 70 hours may be in general mental health subjects not specific to addiction.
Must be a Certified Addiction Practitioner Assistant or obtain certification within 6 months of hire date.

The Intake Specialist facilitates the registration process of clients for clinical service. This person must be able to pay very close attention to detail and get the required information from clients and families.
Collects client data to help with the admission process and ensures that all forms are properly signed and filed. Processes paperwork for new clients efficiently and in accordance with policy and procedures including all intake paperwork and releases of information.
Screen clients for alternative health resources and benefits; verifies 3rd party payers.
Screens clients for further assessment of substance use disorders utilizing a Simple Screening Instrument of Substance Abuse.
Answers and screens inquiry calls from prospective clients.
Uploads all information into the electronic health record (EHR) system, ACCUCARE.

Responsible for the supervision of the Transitional Home during the weekend. The Weekend Supervisor will provide a safe, trauma informed, supportive environment for all residents.
Supervise staff and maintain a schedule for all weekend employees.
Treat all clients with dignity and respect and work with residents to provide support, encouragement, and guidance.
Accurately report incidents or any behavior that includes significance on provided forms.
Assist and supervise clients with preparation of meals.
Assist clients with scheduling appointments, chores, community service and other activities.
Assist in the transportation of clients, adhering to all traffic and safety laws.
Communicate difficulties and or concerns regarding client’s behaviors to the supervisor as appropriate.
Ensure shift chores are completed, especially the deep cleaning of the facility.
Conduct alcohol and drug screening in conjunction with visits and employment.

Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.
Completes case management tasks as necessary.

The Transitional House Manager is responsible for assisting in the oversight and management of the White Buffalo Recovery Transitional House. This facility is an ASAM Level 3.1 Community Transitional Home operated in Arapahoe.
Supervise all direct staff and operational oversight including program and services development and support for the Transitional House.
Implementation of system-level strategic plans, policies and procedures.
Works daily with residents to provide support, encouragement, and guidance.
Ensuring efficient and effective daily operations and delivery of professional services in a safe and secure recovery environment.
Assists in coordination of crisis and emergency situations with the clinical staff.
