The position will report to the CEO, Wind River Family and Community Health Care System. The Division of Early Childhood Healthy Care and Development will enable the utilization of Arapaho Culture and Language as a component of viable health promotion and prevention. The Administrator will incorporate the most current and effective approaches in early childhood learning through immersive tribal language and culture in the child care and early learning services offered by WRFCHCS. This approach is designed to reduce health disparities and address the social determinants of health.
The Administrator is responsible for assisting with the development and delivery of an immersive early childhood learning center that promotes health and child development and revitalization of the Arapaho language among the children and their families. The Administrator will work a team-based approach for assisting the child care and early learning center administrators/supervisors/staff and as the liaison between their work/services and the services offered by and through WRFCHCS. The Administrator will assist in development of direct and adjunct services offered under the Division and serve as the liaison between the child care/early learning services and the various WRFCHCS and community partnerships.
Coordinates the recruitment of new students and conducts informational sessions at schools and in the community for prospective families.
Coordinates with community child education and development programs to ensure articulation of programs design and curriculum.
Communicates frequently with parents to update them on Language, Education, and Wellness activities, to identify community resources and to help parents support their students’ academic growth.
Works with child care/learning center administrators/supervisors to adopt criteria for teacher certification, recruitment and hiring and serves on interview committees.
Develops and implements a certification and other training programs direct providers of child care/early learning services on the adopted, best practice education model and approach, which incorporates Arapaho language and cultural immersion in promotion of healthy child development.
Develops a list of community language resources (e.g. available speakers, venues for field trips) that can facilitate authentic use of Arapaho language.
Develops an understanding of curriculum that will be used in the Language Immersion curriculum, and researches supplemental materials and resources.
Creates and monitors procedures for staff to collaborate on planning and implementing the instructional program.
Stays current on research and best practices related to the education of language learners, attend trainings and conferences to build knowledge.
Engages in networking with school districts for grade advancement or promotion.
Ensures that program communications are up-to-date (website, video, brochure)
Communicates with WRCHCS and other staff to advocate for the needs of the students
Responsible for implementation and utilization of database for tracking student data, creation of timelines for reporting, and providing evaluation of outputs and outcomes for the CEO, WRFCHCS.
Attend and participate in all in-services, staff meetings, and parent council meetings
Offer input to the Director, and or Curriculum Materials Developer regarding activities and materials for the classroom.
Other duties as assigned to maintain and enhance the operation of the department.
Knowledge of Arapaho Language and culture and a continuous willing
Degree in Indigenous Studies or equivalent education.
Knowledge of the Arapaho language and culture is an asset.
Minimum four years of experience in project management, outreach, and community engagement.
Understanding of Arapaho culture and values.
Knowledge and understanding of Arapaho Principles.
Knowledge of the needs of the community
Knowledge of community services and supports.
Knowledge of early childhood development preferred

The Arapaho Language Teacher Assistant position is responsible for assisting the Teacher with the development and implementation of a quality preschool language immersion education program. This position requires the Arapaho language to be spoken exclusively at all times to the children.
Assist the Head Teacher with all preschool language immersion program including:
Ensure to only speak the Arapaho language to the children attending the program
Interact with children throughout the day through the Arapaho language
Carry out all classroom activities in accordance with curriculum adapted for the immersion program
Guide children in a positive manner
Know the procedures to follow in case of an emergency
Present audio, visual aides and other materials to the children
Help idle children find an activity
Help children with their activity
Encourage children to sit and behave properly
Assist if necessary, in serving lunch and snacks to the children
Help children wash hands and take care of toilet needs
Keep the room orderly, clean, and safe
Plan, coordinate, implement, chaperone field trips, outings, and nature walks in conjunction with curriculum activities
Be responsible for the safety and physical well-being of the children at all times
File and record weekly records for each child’s language development on the “Language Development Record Sheet”
File and schedule evaluations for each student and keep child’s file updated
Attend and participate in all in-services, staff meetings, and parent council meetings

Perform laundry duties associated with classroom and student needs

Provides intakes, assessments, and treatment planning for patient care.

Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff.
Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care as required.
Participates with other medical professionals in educating the community and medical staff on mental health issues, including crisis intervention.
Work to ensure that documentation requirements are met to maximize billing and provide best clinical practice.
Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, update clinical record, including progress notes and other relevant information, and enters the necessary information into the electronic health record system.
Collaborates with WHRFCHC and other Tribal departments for continuity of care.
Performs other duties as assigned to maintain and enhance the operation of the program.
Masters’ level education or higher in Social Work, Professional Counseling, Marriage, and Family Therapy

Behavioral Health Quality Assurance & Administrative Assistant serves as a member of the team in the outpatient clinic at the Wind River Family & Community Health Care Center and assists with quality assurance and administrative tasks for the department.
Review charts of patients seen by Behavioral Health Providers the previous week to ensure that all patients have up to date treatment plans and intakes and develop reports for each clinician these are not completed.
Review intake paperwork to ensure all signatures are provided.
Will be Behavioral Health Departments point of contact for ordering supplies in Workflow.
Ensure that Behavioral Health employee timesheets are completed.
Assists the Behavioral Health Director in scheduling performance evaluations are completed for Behavioral Health Employees.
Assign behavioral health patients and ensure that clinician has completed documentation showing that patient has been followed up with.
Answer the Behavioral Health Department phone and forward messages to the appropriate clinician.
Assist in coordinating outside referrals.
Work with providers to ensure patient schedules are correct.
Perform other duties as assigned by the Behavioral Health Program Director.

Provides mental health case management to patient of the behavioral health departments. Case manager will follow up, monitor, participate in crisis management, and connect with other services.
Provides case management services consisting of:
Linkage – Working with clients and/or service providers to secure access to needed services. Activities include communication with agencies to arrange for appointments or services following the initial referral process and preparing clients for these appointments. Contact with hospitalized clients, hospital/institution staff, and/or collateral to facilitate the client’s reintegration into the community.
Monitoring/Follow-Up – Contacting the client or others to ensure that a client is following a prescribed service plan and monitoring the progress and impact of that plan.
Referral – Arranging initial appointments for clients with service providers or informing clients of services available, addresses, and telephone numbers of agencies providing services.
Advocacy – Advocacy on behalf of a specific client for the purpose of accessing needed services. Activities may include making and receiving telephone calls and the completion of forms, applications, and reports which assist the client in accessing needed services.
Crisis Intervention – Crisis intervention and stabilization are provided in situations requiring immediate attention/resolution for a specific client. The case manager may provide the initial intervention in a crisis and would assist the client in gaining access to other needed crisis services.
Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, up to date clinical records, including progress notes and other relevant information, and enter the necessary information into the electronic health record system.
Adheres and abides by rules of Wyoming Professional Licensing Board and applicable Codes of Ethics for profession.

Psychologists will assist the Behavioral Health Department by providing both psychological evaluations and consulting to behavioral health clinicians as well as provide individual and group mental health therapy.
Responds to referrals from the Behavioral Health and Medical Providers
Provide competent and prompt psychological evaluations to children, adolescent, and adult patients to facilitate the identifying of needed resources and determining treatment recommendations.
Provide psychological testing within their scope of practice and completes reports in timely manner.
Provides direct patient care to diagnose and treat patients with a wide range of psychiatric disorders, as well as perform mental health evaluations.
Provides clinical support for the Medical Department and provides consultation services on mental health matters to other WRFCHC professional and allied staff.
Refer patients for admission to acute psychiatric inpatient settings when level of care cannot be provided in-house. Coordinates all such referrals and ensures provision of timely follow-up care.
Participates with other medical professionals in educating the community and medical staff on mental health issues.
Complete all documentation within one week from the date the services were provided to maintain quality and productivity standards, update clinical record, including progress notes and other relevant information, and enters the necessary information into the electronic health record system.
Collaborates with WHRFCHC and other Tribal departments for continuity of care.
As a team member develop productive and cooperative working relationships with health care providers within the facility as well as allied healthcare and social service providers.

Functions as a member of the clinical team, assessing nutritional needs of WRFCHC patients. This includes, but is not limited to planning, coordinating, and evaluating the nutritional component of health care services for specific comorbidities and throughout the life span.
Works independently and in collaboration with WRFCHC medical team(s) and community outreach programs.
Conducts nutritional assessment and provides Medical Nutrition Therapy (MNT) based on disease process, using the Nutrition Care Process (NCP). Disease processes may include but are not limited to prediabetes, diabetes, gestational diabetes, diabetes during pregnancy, adult obesity, childhood obesity, malnutrition, chronic kidney disease, hypertension, and hyperlipidemia.
Provides diabetes nutrition counseling/education to the patients and their families that are consistent with cultural, social, and physiological requirements.
Works with the diabetes educator/coordinator for each facility in coordinating services aimed at reducing the incidence of diabetes, assists in maintaining the diabetes registry assuring all data is current and accurate, analyzes and interprets diabetes data, compiles diabetes-related reports including diabetes audit and cost and care report.
Responsible for educating diabetic patients about disease, process, management, complications, and treatment using the Association of Diabetes Care and Education Specialists (ADCES).
Assist the diabetes educator/coordinator in coordinating all preventive care for the diabetic patients, including, but not limited to annual labs, EKG, retinal eye exams, dental exams, etc.
Observe patients, record significant conditions and reactions, and notify the supervisor or primary care provider (PCP) of the patient’s condition and reaction to drugs, treatments, and significant incidents of in-person visits.
Initiates a patient education plan according to the individualized needs of the patient, as prescribed by the PCP including patient and family instruction.
Participates in department or WRFCHC quality improvement activities as needed.
Keep up with or contribute to the latest food and nutritional science research.

The Dialysis Patient Care Technician Position provides safe and acceptable dialysis treatment under the supervision of a registered nurse. Performs basic patient care activities.
Observes patients and reports unusual conditions and/or reactions to RN.

Prepare machines for hemodialysis treatments.
Obtain pre and post treatment weights of patients per protocols.
Safely initiate and terminate dialysis treatments per center protocol, and within scope of practice and as allowed by state regulations.
Monitor patients during dialysis treatments.
Accurately document patient vitals, treatment data, and any changes or concerns in patient records.
Mix acids and bicarbonate according to protocol.
Complete water testing on a dialysis machine at the beginning of each treatment day and as directed by the Administrator. Complete/document quality control logs throughout the day and after all treatments, as required.
Responds to the patient’s needs with compassion and concern.
Participate in staff meetings as required including in-service and continuing education that are offered for certifications.

Forwarding of mandatory grant training information to program staff and tracking of attendance per grant requirements.

The coder position is located in the Health Information Management Department. The purpose of this position is to perform quantitative and qualitative analysis of the health record, ensure accurate and timely documentation by the medical and clinical staff. This position reports to the Health Information Director.
Performs highly technical and specialized functions for the Wind River Family and Community Health Care. Analyzes and codes diagnostic and procedural information that determines Medicare, Medicaid and private insurance programs.
Quantitative analysis: performs a comprehensive review for the health records to assure the presence of all component parts such as patient and record identification, signatures, and dates where required, and other necessary data in the presence of all reports which appear to be indicated by the nature of the treatment rendered.
Qualitative analysis: evaluates the health record for documentation consistency and adequacy. Ensures that the final diagnosis accurately reflects the care and treatment rendered. Reviews the health record for compliance with established third party reimbursement agencies and special screening criteria.
Analyzes provider documentation to assure appropriate evaluation and management (E & M) levels are assigned using the correct CPT Code.

The Medical Record Technician is located in the Health Information Management department. Performs accurate and timely documentation of histories, subjective findings, physical exams, plans, and prescriptions with timely follow-up in EHR after the visit.

This position is responsible for screening, testing, evaluating, diagnosing and treatment of injuries, diseases, and disabilities using physical therapy procedures and modalities in accordance with standard physical therapy practices.
Consult with patients to learn about their physical condition, symptoms, review medical history and referrals from doctors or surgeons.
Performs subjective and objective evaluation according to diagnosis, history, and physician referral while developing a plan of care with appropriate goals and objectives based on patient evaluation.
Coordinate care with the primary care provider for prescribing medications to relieve pain and increase mobility.
Diagnose movement dysfunction by listening to patients and observing their movements.
Develop individualized treatment plans using various treatment techniques.
Coach patients and teach them therapeutic exercises and stretches to improve their condition.
Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing.
Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly.
Advise patients and their families on in-home treatment options and what to expect from the recovery process.
Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and other clerical duties.
Document patient care in accordance with regulatory, licensing, payer, and accrediting requirements.
Instruct the patient’s family, care team, and physical therapy assistant to follow through programs and exercises.
Maintain equipment work areas in a safe and clean condition.
Supervise Physical Therapy Assistants in accordance with licensing and regulatory requirements and WRFCHC policies.

Keep the therapy area clean and organized.

Performs tasks involving physical labor for installation of components for heavy utility construction projects. May operate hand and power tools of all types: air hammers, earth tampers, cement mixers, small mechanical hoists, surveying and measuring equipment and a variety of other equipment and instruments. Clean and prepare job sites, dig trenches, set braces to support sides of excavations, and clean up rubble and debris.
Install sewer, water, and storm drain pipes using, using pipe-laying machinery and laser guidance equipment.
Dig trenches during excavation operations using a shovel, rake and various digging tools.
Assemble and disassemble construction equipment like scaffold, ramps and braces.
Assist in the loading and unloading of equipment and raw materials.
While performing duties of this job, the employee will be exposed to moving mechanical parts and outside weather conditions.

Certified Addiction Practitioner (CAP) is a person certified to practice addictions treatment, prevention, intervention, diagnosis, referral and follow up under the supervision of a qualified clinical supervisor.
Completes Addiction Severity Index (ASI) assessment to determine appropriate ASAM Level of Care.
Develops individualized treatment plans with patients based on ASI assessment and ASAM Level of Care Recommendations.
Conducts individual, group, and family substance abuse therapy with patients based on ASAM Level of Care Recommendations.
Completes case management tasks as necessary.
Completes necessary documentation according to CARF accreditation and 3rd party payer standards.
Completes paperwork requirements to maintain quality, up-to-date records, including progress notes and other relevant information, enter the necessary information into the electronic health record (EHR) system, ACCUCARE.
Ensure all clinical documentation, services and issues will be reviewed with a designated qualified Clinical Supervisor a minimum of 1 hour per week for 20 hours of direct clinical care.
Adheres to NAADAC Code of Ethics, as well as the Wind River Family and Community Health Care and White Buffalo Recovery Center policies and procedures
Collaborates with WHRFCHC and other Tribal departments for continuity of care.

Bachelor’s degree in addictions, counseling, chemical dependency, or related field.

First Aid/AED/CPR within 60 days of hire.

The Facilities Technician will provide standardized cleaning and will perform a variety of duties involved in the preventative maintenance of facilities and maintenance/housekeeping to the buildings, infrastructure, and grounds.
Painting: Performs spray or brush painting of walls, ceilings, exterior buildings, doors, window frames, siding, and trim. Masks surface preparatory to painting. Applies specialized coatings to roofs.
Carpentry: Perform rough or simple type carpentry in the repair and maintenance of buildings and related furnishing or structures. Duties may include framing, box construction, and concrete forms making minor repairs such as replacing broken boards, nailing down loose wall siding and caulking doors and windowsills.
Performs all tasks in a professional manner to reflect the highest integrity of the department to ensure the continuity and efficiency of patient care and daily operations.
Routine cleaning of gym, lobby, classrooms, kitchen and bathrooms in a clean, neat, and sanitary manner.
Routine cleaning of high traffic areas to include walls, cabinet surfaces, tables, stands and any other furniture and areas as directed.
Cleaning and maintenance of all floors; carpet, tile, brick, and concrete flooring.
Completes trash removal to include biohazard bags to appropriate disposal area.
Ensure all areas are adequately stocked with supplies, paper products, and other essential items required to keep all areas functioning.
Utilizes the appropriate chemicals, supplies, paper products, and other essential items required to keep all areas functioning.
Snow and ice removal.
Report safety hazards as appropriate.
Replenishes supplies; completes monthly supply order and submits to Youth Wellness Director and Custodial Services Supervisor.
Follows all infection control practices including washing techniques, standard precautions, and isolation precautions.
Observes safety precautions at all times.
Other duties assigned to maintain and enhance the operation of the department.
High School Diploma or equivalent required.
Must possess a valid driver license and be eligible for coverage under WRFCHC auto insurance policy.
Experience in custodial or janitorial work.
Knowledge of chemicals, cleaning techniques, equipment operation, and safety protocols.
Physical stamina, ability to stand for extended period of time, and perform repetitive tasks.
Ability to lift and move heavy objects.
Must be able to pass a criminal background check under the Indian Child Protection and Family Violence Prevention Act (P.L.101-630).
