The Transportation Department at Wind River Family and Community Health Care believes in providing safe and secure transportation services to our community. We provide transportation for our patients to their appointments within our facilities and to outside provider-referred specialty clinics. We are here to help with providing consistent and reliable transportation services with excellent customer service. We have handicap-accessible vans to help our patients with altered mobility status. How to Schedule Your Pickup
Call Wind River Family and Community Health Care at 307-856-9281 and select option 6. You will then be transferred to one of our friendly dispatchers. Your transportation referral information will be taken. Your provider or nurse can also make a transportation referral on your behalf. Please double check with the dispatchers to ensure you are on the schedule.
What Information Will the Discpatcher Need?
Name of patient, date of birth, phone number, pick up address, drop off address, mobility status (handicapped, use of assistive devices, etc.), need for car seats or booster seats (for children per State regulations), how many people will be riding in the vehicle, appointment information with date, time, provider, and the facility you are scheduled for.
We currently offer transportation services to all Wind River Family and Community Health Care clinics, and outside provider referred specialty clinics across the state of Wyoming, and out-of-state to Utah, Colorado, and Montana. Provider referred appointments scheduled outside of Fremont county and to out-of-state clinics require a transportation referral to be submitted two weeks in advance.
Drugs, alcohol, and weapons are strictly prohibited in our vehicles. We will not compromise the safety of our riders, drivers, or the organization. No person(s) under the influence of drugs or alcohol may ride in the vehicles to ensure the safety of our drivers and passengers. There is a limit of passengers depending on the vehicle being used. We request that caregivers or family members assist the patient with getting in and out of the vehicle and their belongings. We are required to follow all state and federal regulations regarding traffic laws, safety restraints, and safety practices. Due to high demand in this department, our drivers can only wait 5-10 minutes for the patient to get to the vehicle for transport. We request that you cancel your transportation services (pick up) at least two hours prior to your pick up time. Failure to cancel your transportation services can result in a no call/no show. Three no call/no shows will result in a 30-day ban of transportation services. We request that all riders, family members, and caregivers treat our staff with the same courtesy, dignity and respect that our drivers and staff members treat them with.
