JOB SUMMARY:

The purpose of this position is to perform a full range of Public Health Nurse duties in accordance with established Wind River Family and Community Health Care policies, procedures, and methods.


DUTIES AND RESPONSIBILITIES:

Assesses patient needs and then develops plans of care. Consults with health care team members including participating in case management with health care providers.
Administers the therapeutic measures as prescribed by the health team, evaluates, and reassesses patient needs.
Shares health information and collaborates with patients/families and other community resources. Provides health education to individuals, families, and community groups in various settings, i.e., schools, correctional facilities, community clinics, and other health-care settings.
Educates and coaches people to make appropriate and informed decisions about health issues.
Works with others to identify high-risk patient health problems and diagnoses. Collaborates with other health care professionals to systematically track, provide services, and monitor these populations.
Performs segments of community assessments to develop a plan of action. Conducts community analysis in partnership with community representatives, and in collaboration with other public health or health care officials and professionals.
Tracks community health status evaluates health trends and risk factors of population groups. Collects data in a systematic and organized manner, i.e., demographic, immunization, and analyzes the assessment data. Partners with community leaders to evaluate the data and determine opportunities and needs for the future.
Investigates health problems and hazards such as communicable diseases and develops a recommended plan of action to address these issues.
Consults, advises, and collaborates with individuals, families, communities, and local, state, and Federal systems. Helps to determine the priorities for the targeted interventions. Recommends interventions to mobilize resources for action.
Ensures that current epidemiological processes are followed. Conducts epidemiological surveys, field investigations, and research studies.
Assess and determine needs working in collaboration with a variety of other organizations.
Develop and participate in community health events and mass immunization clinics throughout the Northern Arapaho Community. Settings will include schools, senior centers, community centers, and various other indoor and outdoor public locations within all our communities.
Collect data during screenings and immunization events; utilize this information to determine other potential community needs or public health issues that PHNs can participate in to help improve the health of the community.
Performs continuous evaluation of program and services, and performance improvement. Develops Standards of Care and evaluates outcomes of interventions to continually improve population-based care.
Other duties as assigned to maintain and enhance the operation of the department.


QUALIFICATIONS:

Must be a graduate of an accredited university or college; a Bachelor of Science in Nursing (BSN) is preferred; however, an associate degree in nursing (ADN) may be accepted with the goal of obtaining BSN within 2 years of employment or less.
All nurses must be licensed as a Registered Nurse and provide evidence supporting certifications such as BLS and other necessary competencies (e.g., ACLS, TNCC, PALS, etc.).
Must have computer skills for the application of the EHR software.
Must possess a valid driver’s license and be insurable under WRFCHC’s insurance carrier.
Must be able to pass a criminal background check under the Indian Child Protection Act and Family Act (P.L.101-630).
