The Community Specialist, (CS) will conduct intake and triage for immediate medical and behavioral health needs of children and youth; assessing strengths, risks and needs and performing biopsychosocial histories and screenings for youth referred to the PTI Healing Center. This includes completion of checklists, summary reports, data entry and assistance in the creation of case plans. These duties are in addition to others that may be assigned if qualified, such as providing case management, peer specialist services or other ancillary support services to groups or individually assigned youth, and implementation of evidence based and culturally based best practice programs.
Essential Job Functions include, but are not limited to:
Assists in organizing service and supports an integrated system of behavioral health and other health care for out of home placed, court involved and at-risk children and youth.
Establishes and maintains quality relationships with agencies, community agencies and other resources to facilitate appropriate referral recommendations.
Document all contacts and services in narrative form, enter data into the appropriate recording devices/databases and obtain all required supervisory signatures for service delivery.
Assist patients in accessing support and services as specified on individual plans of care.
Assist in facilitating education groups, or if certification allows, therapy groups.
Provides input and recommendations for treatment planning.
Work to resolve conflicts at the team level and effectively use supervision and management for further conflict resolution.
Attends all mandatory staff meetings, and adheres to all WRFCHC policies, procedures and employment practices.
Other duties as assigned.
KNOWLEDGE REQUIRED BY THE POSITION
Knowledgeable of the people, traditions, programs and governments of the Wind River Indian Reservation Willingness to learn and to be challenged.
Must have good stamina, and be able to think out of the box.
Mature and responsible.
The position involves working with multi-problem children or families with complex needs which is often crisis oriented and unpredictable.
The CS shall demonstrate the highest standard of personal integrity, honesty, confidentiality and truthfulness in all professional and personal dealings.
The CS shall promptly report any conflict of interest to their supervisor concerning anyone coming before them for care.
The CS must be comfortable dealing with victims of child abuse, neglect and other crimes against persons and those with addictions, and be able to treat all in a non-judgmental manner.
Must have a valid driver's license and be insurable.
Must be able to pass a drug test.
Must be able to pass a background investigation
EDUCATION AND EXPERIENCE
Bachelor in Social Work, certification preferred or to be sought as part of employment, and experience in the field of child/youth trauma; or Certified Mental Health Worker, or Certified Peer Specialist with 2-5 years peer specialist experience,, or previous employment as a child or youth worker with 5 or more years of experience in case management or direct care plan services delivery to children/youth in in the social work, health or juvenile justice field.
Alternatively, a Registered Nurse with 2 years pediatric trauma service experience or an Associates degree in criminal justice, human services or early childhood education, and 2 years related experience in case management or direct care delivery. Training will be required and provided to obtain certification as a social worker and/or community health worker as a part of acceptance of a Community Specialist/Youth position.
The applicant must have 2 years of continuous recovery from addictive substance use and behaviors.
Guidelines include WRFCHCS policies, procedures, and manuals, as well as federal and state regulations, which may have gaps in specificity and protocols recommended by The National Child Advocacy Center for child advocacy center. Judgment is required in applying the guidelines and making minor deviations to adapt the guidelines to specific cases. The CS will perform the general and specific duties of the position, and maintain strict confidentiality of all client/patient information, including all requirements of the Patient Protection and Accountability Act and Health Information Patient Protection Act and established policies. Familiarity with or willingness to learn tribal laws and Minor in Need of Care and Juvenile Court procedures, Individual Education and 504 Act plans, and Mental Health, Development and Risk Assessment instruments .
