Wind River Family & Community Health Care is seeking applicants for the position of Clinical Care Team Leader. Wind River Family & Community Health Care provides ambulatory health care for the service population of the Wind River Reservation. The Clinical Care Team Leader serves as a member of the clinical staff of the ambulatory health care clinic located in Arapahoe, Wyoming.
This position serves to facilitate safe, efficient, high-quality care within the Wind River Family and Community Health Care system. This position involves provision of leadership to groups of care team staff, while maintaining “hands on” patient care skills as a member of the care team. Responsibilities include coordinating process improvement initiatives with clinical leadership staff, as well as taking responsibility for the effective organization and delivery of care across all services in their absence.
Works directly with patient care teams to coordinate the activities of patient care within a care team of several providers, nurses, and nursing/medical assistants.
Orients new nursing staff to patient flow, clinical processes, patient care expectations, and quality improvement processes.
Ensures current competency of clinical team members and offers educational activities as needed.
Provides evidence-based guidance to care team members, and facilitates development of integrated care design in accordance with current evidence.
Facilitates daily care team assignments and scheduling.
Acts as a resource for care staff in providing clinical care.
Maintains and demonstrates knowledge of and adheres to clinic and departmental safety regulations.
Performs responsibilities for infection control, hazardous materials, safety, security, and emergency preparedness.
Maintains knowledge of information management concepts (Health Records) and effectively participates in implementing the information management principles and maintains security and confidentiality of data and information.
Additional duties as assigned.
Requires experience in quality improvement, evidence-based patient care, and leadership.
Experience coordinating care in a fast-paced environment.
Ability to multitask and to communicate effectively within a care team.
Ability to communicate patient problems, gain patient cooperation, function in stressful situations, promote interpersonal relations, and provide excellent customer service is essential.
Knowledge and ability to appreciate cultural differences of the Native American population and their effect on health care delivery to patients and their families.
The incumbent must maintain strict confidentially and high ethical standards in performing the position.
The incumbent needs to be respectful, possess cultural awareness and sensitivity, be flexible, and demonstrate sound work ethics.
