Wind River Family & Community Health Care is seeking applicants for the position of Patient Advocate Representative. Wind River Family & Community Health Care provides ambulatory health care for the service population of the Wind River Reservation. The Patient Advocate Representative serves as a member of the support staff of the ambulatory health care clinic located in Arapahoe, Wyoming, and will report directly to the Supervisor of the Behavioral Health Department.
This position as Patient Advocate Representative will be responsible for providing special services that assist patients of the WRFCHC.
The successful candidate will be responsible for maintaining productive communication links with patients and their families.
They will assist with providing public information about the Clinic services in various media formats.
This position will assist patients with the successful utilization of the health care system.
Coordinates employee customer service training/orientation.
Participates in cross cultural communication efforts.
Mediates interaction between patients and facility personnel as needed, and assists with emotional concerns, and provide general oversight of WRFCHC customer relations.
The Patient Advocate Representative will seek out and maintain community resources that could help particular patients with immediate needs, to include emergency food supplies or clothing, social services, guidance to the appropriates agencies and organizations to provide aid and the necessary response to certain circumstances.
The candidate will work closely with all aspects of the clinic to include medical, dental, optometry and behavioral health services.
Time will be spent in the clinic, and it is probable that time will also be spent in the field with clients and developing potential resources for the targeted clientele.
Minimum Required Education and Experience:
A Masters degree in social work or a closely related discipline, or a combination of education and experience to closely approximate that level. At least two years’ experience as a social worker. Ability to work with sensitivity in a cross-cultural environment. Excellent interpersonal skill.
The incumbent must maintain strict confidentiality and high ethical standards in performing the position. The incumbent needs to be respectful, possess cultural awareness and sensitivity, be flexible, and demonstrate sound work ethics.
