Wind River Family & Community Health Care is seeking applicants for the position of Maternal Child Health Nurse, (MCH). Wind River Family & Community Health Care provides ambulatory health care for the service population of the Wind River Reservation. The MCH serves as a member of the medical team in the outpatient clinic/treatment room area at the Wind River Family & Community Health Care Center, located in Arapahoe, Riverton and Ethete Wyoming. Home Visiting is required.


The MCH Nurse will be able to promote maternal self-care and infant care, assess and monitor maternal physical and emotional changes, foster parent infant relationship, assess and advise on safety of infant environment.
Provide assessment and teaching and foster independence with breastfeeding and monitor infant growth and development.
MCH Nurse develops, evaluates and revises patient care goals in conjunction with patient and family.
Communicates with MD, family and interdisciplinary care providers to clarify care and increase awareness s of significant changes in condition in a timely manner
Maintain effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.


Qualifications:

A valid, current, full and unrestricted Professional Nursing License is required.
A Bachelors of Science degree from an accredited School of Nursing preferred, and a minimum of 2-4 years experience working in a progressively more responsible nursing position.


Essential Functions/ Responsibilities

Carries out the Agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.
Interprets and implements the Agency’s philosophy to staff and members of the community.
Assumes responsibility for assignments given, seeks supervision appropriately, and is accountable for his/her actions by performing within the limits of his/her education and experience.
Knows and conforms to the law governing the practice of professional nursing and provides professional nursing care using all elements of the nursing process.
Assesses and evaluates the health care needs of patients and families with consideration regarding physiological, psychological, social and environmental factors.
Develops a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community and agency resources.
Implements the care plan and revises it whenever necessary by regularly assessing, observing and evaluating the patient’s condition, needs and response to care, and makes appropriate nursing judgments and decisions for care plan revision.
Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques.
Administers medications and treatments as prescribed by the physician and performs nursing procedures and techniques.
Teaches the patient and family/caregivers self-techniques whenever appropriate and provides instruction regarding medication, diet, safety and treatment modalities in accordance with the plan of care.
Recognizes and utilizes additional opportunities for health counseling/education with patients, families and other caregivers and provides them with information that will facilitate decisions regarding the promotion, maintenance and restoration of health.
Delegates responsibility appropriately, and supervises ancillary personnel in a manner that will assure quality care and compliance with the care plan.
Evaluates for and promotes a safe environment for the patient.
Completes, maintains and submits all required documentation that is timely, accurate and relevant.
Communicates with other health care professionals, both internal and external, to the organization in a manner that assures care plan coordination as well as continuity of care.
Assumes responsibility for personal growth and development; maintains and upgrades professional knowledge and practice skills through attendance/participation in continuing and in-service education programs.
As requested by supervision, participates in and assists with staff and student orientation and development; participates in internal and external committee activities; and represents the Agency in the community.
Participates in performance improvement activities, as required.
Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.
Performs complete prenatal assessment and utilizes appropriate documentation.
Performs complete postpartum assessment and utilizes appropriate documentation.
Performs complete infant assessment and utilizes appropriate documentation.
Establishes and follows plan of care which addresses maternal-child patient problems and needs.
Provides case coordination, health teaching, supportive counseling and advocacy for the patient and family.
Demonstrates proper use and cleaning equipment used in the care of the mother and child
Participate and plans in health promotion/disease prevention activities, including but not limited to health fairs, outreach activities, public speaking, flu and immunization clinics and other activities.
The candidate must possess medical knowledge and skills for completing relative patient care assignments.
Works closely with other members of the nursing team for further development of clinical nursing skills and professional judgment.
Records observations, nursing interventions, therapeutic measures administered, and other data relevant to the patient's care in the medical chart/electronic health record.
Acts as the liaison between the provider and the patient.
Required to maintain BLS and other skills necessary for competence (e.g., ACLS, TNCC, PALS, etc.).
Safely operates specialized equipment. Follows procedures for reporting equipment management problems, failures or user errors.
Maintains and demonstrates knowledge of and adheres to clinic and departmental safety regulations.
Performs responsibilities for infection control, hazardous materials, safety, security, and emergency preparedness.
Demonstrates effective, communication methods and skills, following appropriate lines of authority (chain of command).
Communicates with patients, families and team members to ensure the occurrence of effective therapeutic interventions, and to ensure safe patient care.


Knowledge Required:

Knowledge of and ability to apply professional nursing principles, practices, procedures, and techniques to outpatients with a wide variety of conditions from pediatrics to geriatrics.
Knowledge of medical history taking techniques, physical assessment, and normal body function; disease processes, signs, symptoms, and treatment of acute minor illnesses; pathophysiology of inflammation, infection, allergic reactions, and such diseases as respiratory, gastrointestinal, dermatological, urological, and musculoskeletal conditions, normal and abnormal vital signs, and normal and abnormal neurological signs and symptoms; signs and symptoms indicative of emergent or life threatening nature.
Knowledge and ability necessary to provide care appropriate to the age of patients served.
Incumbent demonstrates knowledge of the principles of growth and development over the lifespan.
Ability to identify and categorize each patient's age-specific grouping of needs, i.e., infant, adolescent, or geriatric patients.
Knowledge of normal ranges of laboratory values and beginning skills in assessment of abnormal radiological findings.
Knowledge of pharmaceuticals, their desired effects, side effects, and complications of their use, and steps to counteract adverse effects in order to properly administer and monitor patients using medications.
Ability to recognize adverse signs and symptoms and to react swiftly in emergency situations.
Knowledge and skill sufficient to provide CPR.
Knowledge and skill to apply ACLS and PALS.
Skill in operating specialized medical equipment such as cardiac monitor, resuscitation devices, etc.
Knowledge of the psychosocial effects on patients within the outpatient setting who have multifaceted problems; of communication techniques to establish rapport and facilitate cooperation and patient compliance in patient education.
Knowledge of communication techniques and ability to teach and instruct patients, family, and/ or significant others in proper health care of patients.
Skill to develop or participate in a nursing plan to meet the needs of assigned patients.
